Please forgive me If this Is not the correct way to respond,but it is the only way I know how to explainthesituation.
At the time you are reading this,a'blnder "with augmented answers to the10 questlans wasprovidedtobothyouandtoJosephTerry,PUC Auditor.I submItted these la goodfalth to show that IsincerelyamwlIHngtocontinuetoworkwithJosephTerryandtoexplainwhathashappened.
I also request that thIs hearIng be dismissed.I do so,because I am 'medicanyhandicapped"with afractureInmyrightfootandcurrentlynotabletodrivetoBolse,I am the only person who representstheCompany.I haveno employeesand I have to outsourcefor malntenance,repa/rs,etc.
Thus,my request.
I Respectfullyrequest the Commissloners of the Idaho ÞublIc Ut/IItles Commission that you pleaseDismIssthisOrdertoappearbeforeyouonFeb22,2023,I have a subchondralfmeture of the talasbone(ankle}on my rightfoot and can't safely drive at this time.I vislted wlth my Doctor today and hehasadvísedthat&Is unsafe to attempt to drive and could result in furtherdamageto my ankle.
I responded to the audit request with answers to the best ofmy abi/Ityto do so at the time.Currently,I have been dealingwith three very challengingmedical issues ,a hematoma blood bleed in my lefthip;extremely diminished vision due to a suddenonset of wet maculardegeneration from a bleed in

my retina.In November of 2022 I was hospitalized for cavid.About 10 days later I was readmittedforextenuatingcircumstancesasaresultofCovId.To help resolveand hopefully maintain and stopfurtherdamagetomyeyesight,i recelveinjections in each eye every few weeks to stop the bleedingandhelprestaremyeyesight.I will haveto have these injections I am told for the rest of my life.It isslowlyrestoringmyvision.
Each of these medical issues contributed to the delay in my responses.I was in contact with ClaireSharpprior,and I was of the understandingthat she was in agreement that extra time was needed.
After she extended the time,I sent the Annual Reports.I sent an email to Joseph Terry with noresponseaftertheseweremail.
I did not anticipate that there would be a(iditional issues.However,with downtime from covid,thehematomableedinBhipandeyes,and then shockingly in January an MRI revealed a fracture In myrighttalas(ankle bone).As I said,In June of 2022 i fell,cut open my left leg,triple spralned my rightankleanditresultedseveralmonthslaterinhematomainmylefthip.I am normally a very healthypersonandthesehavebeen9monthsofhorrlflepolnanddeallngwithdisobilitiesthathavebeenverychallengingforme,I am reassuredby my doctors that in time these wlll hopefully be resolved.Myretinalspecialisthaslimitedmy'screen time to no more than 3 hours per day to help heal my eye.Ofcourse,less would be better,
I sincerely conveythat island Park Water wHI continue to work with Joseph Terryto complete the oudItwiththeunderstandingthattherearetemporarymed/calreasons being dealt with that will requireconsiderationastothenatureoftimelyresponses.
Currently,I am the only person who can appear before you to expla/n the SItuatlon.As prevlouslyexplained,through a series of veryunfortunate accidents starting In June 2022 and resulting med/calevents,currently I am "Medically handIcappeddue to medical issues explained below that maketraveltoBoiseextremelydiff¡cult and III advlsedby my Doctors.
Also,I respectfully request this Order be dismIssed based upon thefact that In goodfaith and to thebestofmyknowledgeandabilitytodoso,I haveprovided o"second response"to thef/rstfor therequestedinformationbyJosephTerry.Thls Information has been provided to both the put -Auditor,Joseph Terryand a secondcopy has beendel¡veredto the Commisslon vla FED-Xdelivered Feb 246 byendofdayperFed-X,
I respectfully submit that thefirstresponse to the audit questlonswas basedupon the fact that I trulybelievedthisInformationwasalreadyonfilewiththePUCfromapreviousauditin2019.Also at thattimeIwastoldtoredactanypersonaldataforsecurityprotection.As you can see from theInformationprovidedtoyou,the only redaction to the bank statements for security and privacyprotectionwasduetothefactthisinformationwasbelngprovidedvlatmallservice.Also,when theinformutlonwassubmitted,o request was noted in the responses to please contact me //furtherInformationwasneeded.This did not happen.
I was aware that the PUC retained copies ofprevlously submitted informotlan due to thefact thatovertheyears,Chris Hetht referred back to such copies at his disposal,I trulybelleved theinformationbeingrequestedwascurrentlyonfilewiththePUC,readily avullable and had beenprovidedontwopreviousoccasionstothePUC.This Is why the questlons were answered that way,

o.Jan 2023 I was diagnosed with an subchondralfracture in the talar head bone(ankle bone)with Indications of sIlght collapse ofthe subchondralboneplate,InmyrIghtfoot.This extremely Ilmits (for the time)any welght bearing on the rightlegandandplacedbackintaafootbrace,whlch makes it both dangerousanddißiculttodrivethelongdistancefromIdahoFa/Is to Boise and limits the amountoftImeIcanputweightonit,
b.In October 2022,I developeda hematoma blood clot that bled out in to my leftleg,hlp ond to my knee,from over use due to the prevlousInjuryfrom afall inJune.The hematoma *blood clot has re-occurred.
c.November of 2022 I was diagnosed with rapId onset of wet macular degenerationInbothofmyeyesbyDr.Ellington,retInal specialist.(there was a bload bleed Intotheretinaofbotheyes)The treatment to save my vlslon and hopefully restore orstabIllzemevislon,requires I be given two shots in each eye -everyfew weeks &repeatedly on going,which wi/I continue until which time my vlslanis stableenoughtoextendtheInjectionsfartherout.
d.Dr.Ellington,Retinal Speciallst treating me,has restricted 'computer time"to lessthan3hrs,per day or less,in order to help restore my VIslon,e.November13,2022 i was hospitallred at Community HospItalin Idaho Falls andwasdIognosedWIthCov/d.I was re-admitted for seriousconsequences of Covidwithin10daysfromthefirstdlagnosisIwasveryi/I,but have now recovered fromthis,with what Is called'Ionghaul cov/d",f.June of 2022 /was serlouslyInjured by a fall on a rock,that caused a deepcut Inmyleftleg,and a triple sprain in my right ankle that restr/cted my ability to walkformanymonthswithoutascooter,cane and brace.Due to the strain ofusing theleftlegtohelpwIththerightlegspraln,It resulted In a suddenHemotomo In mylefthlp.This blood clot bleed spreadfrom my left hip to my left leg with a verydangerousbleed,which also caused difficulty in walking on my leftleg,
I havebeen receiving therapy three times/week at Rehab Authorityfor many months,which hasgreatlyhelpedbutst/Il am basicaky handicapped unt/I henled.It was shock/ng to discoverthat InJanuarytherewasapreviouslymisdiagnosedInterlorfractureInthetalasbone*ankle bone in myrightfoot,which has now been restr/cted to Ilm/ted welght bearIng In order to try to heal,to preventthetalasbonefromcrumbHng.If the bone doesn't heal,to save myfoot,futuresurgery may herequired.Time will teu,as it con take upwards of a year or more for this type offracture to heal I amadvised.
I am ossured that with potlence and time,and good medical I wHI recover.
