M-8192-3 AFFIDAVIT OF SERVICE Daniel L. Vickers, being first duly sworn, states under oath as follows: 1.  I am employed as an investigator for the Motor Carrier Division of the Public Utilities Commission and I have personal knowledge of the events stated in this affidavit. 2.  On September 18, 1996, I was in the Sun Valley/Ketchum, Idaho area as part of my duties as a Motor Carrier Division employee.  On that date, I received from the Secretary of the Motor Carrier Division a photocopy of Order No. 26616, Order to Show Cause, and a Notice of Hearing in Case No. M-8192-3.  Order No. 26616 and the Notice of Hearing were delivered to me in order that I might attempt personal service of the documents on Christopher B.  Simpson and Simps 4, Inc. doing business as Bald Mountain Taxi.   3.  On the morning of September 19, 1996, at approximately 8:15 a.m. and again at approximately 9:00 a.m., I went to the office of Bald Mountain Taxi at the corner of Sun Valley Road and Spruce Road in Ketchum, Idaho.  On both occasions, the office door was locked and no one was present. 4.  At approximately 9:45 a.m. on September 19, 1996, I obtained an address for the residence of Christopher B. Simpson from the Bellevue, Idaho marshalls office.  The address provided to me was 204 Gannett Road in Bellevue, Idaho.   5.  At approximately 10:15 a.m. I arrived at 204 Gannett Road in Bellevue, Idaho to attempt personal service of the Notice of Hearing and Order No. 26616 on Christopher B.  Simpson.  No one responded to my knocks on the front door and pressing of the door bell. 6.  After leaving the residence at Gannett Road, I determined to return to the Ketchum office of Bald Mountain Taxi.  However, while traveling to Ketchum, I saw a Bald Mountain Taxi van traveling in the opposite direction.  I then followed the Bald Mountain Taxi van which stopped at KSKI radio station located between Ketchum and Hailey, Idaho on State Highway 75.  At approximately 11:00 a.m. on September 19, 1996, I served the copy of Order No. 26616 and Notice of Hearing on the driver of the Bald Mountain Taxi van, Donna Perry.  Perry indicated to me that she would see Christopher Simpson in approximately one hour and would deliver the documents to him.   Further your affiant saith naught.
