Qwest Tariff Advice 01-04-S

Attached is a Memorandum from Wayne Hart, Telecommunications Analyst, regarding his request for approval of Qwest's tariff advice to enhance Centrex 21 service. As the Memo explains, the Company requested a less than 30-day tariff filing notification since the filing is an enhancement to the Centrex 21 Service and does not impact the monthly rate. Staff agrees with the Company's claim that the changes can be considered an enhancement to the Centrex 21 product and recommends this tariff advice be approved by Minute Entry.

Therefore, Tariff Advice 01-04-S to enhance Centrex 21 Service is hereby approved this date.
