Effective April 1,2011,Qwest Corporation registered the fictitious name
CenturyLink QC.Effective August 8,2011,Qwest Corporation began operating
under the name CenturyLink QC.As such,Qwest Corporation d/b/a
CenturyLink QC hereby adopts,ratifies and makes its own,in every respect as if
the same had been originally filed by it,all schedules,rules,notices,
concurrences,schedule agreements,divisions,authorities or other instruments
whatsoever,filed with the Idaho Public Utilities Commission,State of idaho,by
or adopted by Qwest Corporation.
By this notice,Qwest Corporation dib/a CenturyLink QC also adopts and ratifies
all supplements or amendments to any of the above schedules,etc.,which Qwest
Corporation has heretofore filed with said Commission.(N)

1. APPLICATION AND EXCEPTIONS 1.1 APPLICATION OF TARIFF This regulations, rates and charges applicable to the provision of Special Construction of Facilities provided by Qwest Corporation d/b/a CenturyLink QC, hereinafter referred to as the Company to customers, are the same as those filed in CenturyLink Operating Companies F.C.C. Tariff No. 12, (F.C.C. No. 12) with the exceptions listed herein.. The tariffed and regulated services offered herein by Qwest Corporation d/b/a CenturyLink QC, whether under that name or the trade or brand name CenturyLink, are subject to the terms and conditions of this Tariff. The provision of such service by the Company as set forth in F.C.C. No. 12, with such exception listed in 1.2, following, is specifically intended to provide Special Construction of facilities in accordance with an order for service to satisfy that request. The Company will determine, based on the customer's use of the facilities involved, whether Special Construction as specified in F.C.C. No. 12, or rates and charges specified in other tariffs of the Company apply. The provision of such services by the Company as set forth in F.C.C. No. 12 does not constitute a joint undertaking with the customer for the furnishing of any service. The regulations, rates and charges contained in F.C.C. No. 12 are in addition to the applicable regulations, rates and charges specified in other tariffs of the Company. Whenever reference is made in this Company's tariffs to other tariffs of the Company, the reference is to the tariffs in force as of the effective date of those tariffs, and to amendments thereto and successive issues thereof. References to F.C.C. No. 12, Special Construction, F.C.C. Tariff No. 11, Access Service, and NECA F.C.C. Tariff No. 4, V & H Coordinates, will be to those filed with the Federal Communications Commission. References will be to the tariffs in force, and to amendments thereto and successive issues thereof. References made in this Tariff to the term "Interstate" hereinafter becomes "Intrastate".
