1.)Definitions, Tenns and Abbreviations
Company - Whenever used in this price list, Company refers to Inland Long Distance
Company unless otherwise specified or clearly indicated by the context.
Customer - The person, finn, corporation or governmental unit which orders Service
either for its own use as a resale carrier or as a nonprofit manager of a sharing group and
which is responsible for the payment of charges and for compliance with Company price
list regulations. The tenD Customer also includes an entity that remains presubscribed to
the Company Service after its account(s) is removed from the Company s billing system
subsequently continues to use the Company s network, and is billed by a local exchange
carrier for such use, or otherwise uses Service for which no other Customer is obligated to
compensate Company.
Toll Free Service - Toll free service includes 800, 877 and 888 series numbers as well as
any other future toll free series numbers.

General Rules and Regulations
2.1 Undertaking of Inland Long Distance Company
Inland Long Distance Company services are furnished for intra-LATA and inter-
LATA communications originating and tenninating at specified points within the
state ofIdaho. Any calls originating in Idaho and tenninating outside ofIdaho are
not applicable to this price list tenus, conditions, and rates.
Inland Long Distance Company installs, operates and maintains the
communications services provided herein accordance with the tenus and conditions
set forth in this price list.
The Company s services and resold facilities are provided on a monthly basis
unless otherwise provided, and are available twenty-four hours per day, seven days
per week.
Limitations
2.2.1 Services are offered subject to the availability of the necessary facilities and
equipment and subject to the provisions of this price list.
2.2.2 Inland Long Distance Company reserves the right to discontinue or limit
service when necessitated by conditions beyond its control, or when the
Customer is using service in violation of provisions of the price list, or in
violation of the law.
2.2.3 The Company does not undertake to transmit messages, but offers the use of
its facilities when available, and will not be liable for errors in transmission
or for failure to establish connections.
2.2.4 All services and resold facilities provided under this price list are directly or
indirectly controlled by Inland Long Distance Company. The Customer
may not transfer or assign the use of service or facilities without the express
written consent of the Company. Such transfer or assignment shall only
apply when there is not interruption of the use or location of the service or
facilities.

2.2 Limitations (Cont'd)
2.2.Prior written pennission from the Company is required before any
assignment or transfer. All regulations and conditions contained in this price
list shall apply to all pennitted assignees or transferees, as well as all
conditions of service.
The included price list language does not constitute a detennination by the
Commission that a limitation of liability imposed by the Company should be
upheld in a court of law. Acceptance for filing by the Commission
recognizes that it is a court s responsibility to adjudicate negligence and
consequential damage claims. It is also the court's responsibility to
detennine the validity ofthe exculpatory clause.
2.3 Use
Services provided under this price list may be used for any lawful purpose for which
the service is technically suited.
2.4 Payment for Service
All charges due by the Customer are payable to Inland Long Distance Company or
any agency duly authorized to receive such payments. Any objections to billed
charges must be reported to the Company within thirty (30) days of the invoice date.
Adjustments to the Customer s bills shall be made to the extent that circumstances
exist which reasonably indicate that such changes are appropriate. Once service is
activated, the Customer is liable for the payment of all usage charges for services to
be provided by the Company to the Customer including any fraudulent use, misuse
or abuse of the Customer s Service or Customer provided equipment by third parties
the Customer s employees, or the public. This includes payment for calls or services
that originate at the Customer s number(s), are accepted at the Customer s number(s)
(e.g. collect calls), are billed to the Customer number(s) via third number billing, use
of a calling or travel card, or use of an assigned special billing number or
authorization code to the Customer.
Taxes
All state and local taxes (i., gross receipts tax, sales tax, municipal utilities tax) are
listed as separate line items and are not included in the quoted rates.

Regulatory Fees
All State and Federal Commission ordered rates, fees, and surcharges may be listed
as separate line items and are not included in the quoted rates.
Cancellation
1 For any of the following reasons, the Company may discontinue service upon
at least 10 days' written notice or cancel an application for all services without
incurring any liability. Separate accounts for the same Customer are also subject to
this provision.
1 In the event that a Customer s bill remains unpaid after more than
thirty days following rendition ofthe bill.
1.2 In the event of a violation of any regulation governing the service
under this tariff, when necessitated by conditions beyond the Company
control, a violation of any law, rule, or regulation of any government authority
having jurisdiction over the service.
1.3 Where the Company is prohibited from furnishing services by order of
a court or other government authority having jurisdiction.
7.2 The Company, by written notice to the Customer, may, without incurring any
liability, cancel or suspend the provision of service or equipment for the following
reasons: non-payment of any sum due to the Company from the Customer, whether
pursuant to service offered under this price list or otherwise; or as a result of actions
of a government agency which forces discontinuance of the provision of service or
equipment; or for violation or threatened violation of any of the tenus or conditions
of this price list by the Customer or authorized user; or if the Customer becomes
insolvent or bankrupt, or makes a general assignment for the benefit of creditors or
as otherwise pennitted by this price list. Cancellation will be effective on the date
specified on the written notice. A reasonable attempt will be made within twenty
four (24) hours after the ten (10) day cancellation notice period to personally contact
the Customer.

3 Service may be canceled by the Customer only on not less than 30 days
written notice to the Company. In the event the Company is unable to disconnect
the Customer s access line by the requested cancellation date, the customer will be
responsible for any usage over the line.
Other Rules
The Company reserves the right to discontinue service, limit service, or to impose
requirements on Customers as required to meet changing regulatory rules and
standards of the Idaho Public Utilities Commission.
Timing of Calls
1 Billable time for service is the duration oftime between the called station
answering and the called or calling station disconnecting, provided duration may
be rounded in accordance with specific descriptions in this price list.
9.2 The duration of a call is rated in intervals of the billing increments described
for each service provided in this tariff. If the final interval of a call is less than the
applicable billing increment, it will be rounded up to a full increment for purposes of
billing.
3 Computed usage charges or credits for each call are rounded to the nearest
cent when possible.

Timing of Calls (Cont'
9.4 Applicable rate periods, unless noted otherwise, are indicated below and are
based on the time in which a call is established. In cases where a call begins in one
rate period and continues into another, the rate in effect in each period will apply to
the portion of the call occurring within the applicable rate period.
Day rates apply from 8 a.m. to, but not including, 5 p.m. Monday through Friday
Evening rates apply from 5 p.m. to, but not including, 11 p.m. Sunday through
Friday
Night rates apply from 11 p.m. to, but not including, 8 a.m. seven days a week.
Weekend rates apply from 8 a.m. to, but not including, 11 p.m. Saturday and from 8
m. to, but not including, 5 p.m. on Sunday.
On holidays evening rates apply throughout the day on which the holiday is
observed unless a lower rate would nonnally apply.
Peak rates apply from 8 a.m. to, but not including, 5 p.m. Monday through Friday.
Off Peak rates apply from 5 p.m. to, but not including, 8 a.m. Monday through
Thursday, and between 5 p.m. Friday to, but not including, 8:00 a.m. Monday.

Description of Services Offered
3.1 Message Telecommunications Service (MTS)
1.1 The following MTS service arrangements allows a Customer to originate
Intrastate calls in areas with Equal Access capabilities served by the Company
by presubscribing to its MTS long distance calling service plan.
1 MTS Calling Plan " ANYTIME, ANYWHERE"
Description - Anytime, Anywhere calling service plan is available to
Customers served by any LEC equal access office served by the Company.
Anytime , Anywhere plan offers the Customer a flat rated calling plan which
is non-distance sensitive for all their direct dialed long distance calling. The
calling plan has only one rate for intrastate calls. Charges are not based on
the time period (Peak/Off Peak) in which the call is placed. The calls are
billed with the initial increment of thirty (30) seconds and then each
additional increment in six (6) second increments.

Calling Card Service
1 Description - Calling Card Service is a one-way, dial in or out multipoint
service. Calling Card Customers may originate calls from, and tenninate calls to
every city within Idaho. Subscribers who originate calls can access the Company
Calling Card Platfonn by dialing a toll free access number (ie. 800/888). The
Company s Calling Card Service can be used for all direct dialed interstate and
intrastate long distance calling. This calling service is very simple; it offers
Customers a non-distance sensitive single flat rate per minute regardless of time a day.
Charges are not based on the time period in which the call is placed. The calls are
billed in one (1) minute increments and rounded up to the next full minute. A per call
surcharge will be charged to Customers. Any calls originating from a payphone will
have an additional per call surcharge applied. See Calling Card Usage Charges
Section 5.
3.2 A Customer may subscribe to Calling Card Service without subscribing to other
Company services.

3.4 800 Services
3.4.1 The Company 800 Service is an inbound service originating on feature group
facilities provided by the Local Exchange Carrier (LEC) and tenninating on a regular
residential, business line or a Special Access Line (SAL). This service enables the
Customer to receive 800 service calls at their residence or place of business. All 800
calls are billed at a single flat rate per minute regardless of time of day. 800 calls are
billed in one (1) minute increments and rounded up to the next full minute. 800 calls
are rated by either originating interstate or originating intrastate jurisdiction calls. 800
calls can originate from every city within the u.S. Mainland, including Alaska
Hawaii, Puerto Rico, and U.S Virgin Islands. A monthly recurring charge is assessed
to the Customer and is charged by calendar month. The recurring charge is assessed
per 800 number. Any calls originating from a payphone will have a per call surcharge
applied. See Usage Charges, Section 5.4.
3.4.Usage Charges - Refer to Section 5, Service Charges (5.4.1 & 5.4.2).
3.4.3 The Company reserves the right to require an applicant for the Company 800
Service to supply the following infonnation when requesting service: an initial traffic
forecast, identification of anticipated busy hour, identification of its geographical
marketing target areas, and a schedule of marketing and promotional activities. The
Company may also require that a new traffic forecast be submitted by the Customer
quarterly after service is initiated.
3.4.4 The Company s 800 Service is furnished upon condition that the Customer
contracts for adequate facilities to pennit the use of this service without injurious
effect upon it or any service rendered by the Company. The Company may tenninate
or refuse to furnish 800 Service to any applicant, without incurring any liability and
without notice to the Customer, if the use of the service would interfere with or impair
any service rendered by the Company.

3.4.5 The Customer must obtain an adequate number of access lines for the Company
800 Services to handle the Customer s expected demand in order to prevent
interference or impainnent of this service or any other service provided by the
Company considering: (1) total call volume; (2) average call duration; (3) time-of-day
characteristics; and (4) peak calling period. The Company, without incurring any
liability and without notice to the Customer, may disconnect or refuse to furnish the
Company 800 Service to any Customer that fails to comply with these conditions.
3.4.6 Each 800 Service telephone number must be placed in actual and substantial use
by the Customer. "Substantial use" shall mean a pattern of use that demonstrates an
intent on the Customer s part to employ the number for the purpose for which it was
intended; namely, to allow callers to reach the Customer, as indicated, for example, by
at least 30 average monthly minutes of use or more. Any 800 telephone number
associated with the Company 800 Service that has not been placed in actual and
substantial use during the first sixty (60) day period after service activation may be
redesigned as a spare number in the Company 800 database by the Company upon
written notice to the Customer.
3.4.7 Ifthe Customer requests assignment of a specific 800 Service telephone
number, the Company may require the Customer to submit a number reservation
agreement fonD to the Company. At no time may a Customer have more than ten (10)
numbers reserved. Any reservation shall be for no more than sixty (60) days and shall
be subject to a reservation fee.

3.4.8 Nothing in this Section, or in any other provision of this tariff or in any
marketing materials issued by or on behalf of the Company, shall give any person
including prospective Customers who have reserved an 800 telephone number
hereunder or Customers who subscribe to and use the Company 800 Service or their
transferee or assigns, any ownership interest or proprietary right in any particular 800
number; however, upon placing a number actually and substantially in use, as defined
above, the Company 800 Service Customers do have a controlling interest in the 800
number(s). The Company s 800 Service Customer may retain the use of their 800
number assignments, even following changes in their 800 carrier and/or Responsible
Organization (Resp. Org.
3.4.9 If a Customer places an order for the Company to carry Customer s already
existing 800 number service, the Customer shall provide to the Company the contact
names, telephone number and address of the Customer s Responsible Organization
(Resp. Org.). Upon subscription to the Company 800 Service, the Customer may
execute a Letter of Authorization to transfer Resp. argo responsibility of its 800
number(s) to the Company Resp. argo in writing within 48 hours of the change. The
Customer is responsible for all outstanding indebtedness for services provided by a
previous Resp. argo or 800 service carrier. The Company assumes no responsibility or
liability with respect to any obligations of Customer to such previous service providers
existing at the time of transfer to the Company.
3.4.10 Subject to execution of a Resp. Org. Service Agreement between the Company
and the Customer, unless the Customer requests another Resp. Org., the Company
Resp. Org. functions include: I) search for and reservation of 800 numbers in the
SMS/800; 2) creating and maintaining the 800 number Customer record in the
SMS/800; and 3) provision of a single point of contact for trouble reporting.

3.4.11 Where the Company serves as the Resp. argo for an 800 Service Customer, the
Company will, at the Customer s request, subscribe to 800 Directory Listing for the
800 number(s) assigned to the Customer. A charge for 800 Directory Listings may
apply as set forth in this price list. In the event that a Customer transfers its 800
service to another Resp. Org., the Company shall cease to subscribe to 800 Directory
Listing Service on behalf of the Customer and the Customer is responsible for assuring
that 800 Directory Listing Service is maintained through the new Resp. Org. The
Customer is responsible for payment of any outstanding 800 Directory Listing
charges, including any unexpired portion of any minimum period applicable to such
service, and the Company shall have no liability for any interruption or other delay,
error, mistake, omission or other defect occurring in connection with the transfer of
800 Directory Listing responsibility.
3.4.12 Where the Company serves as the Resp. argo for an 800 Service Customer, it
will, at the Customer s request, subscribe to Vertical Features
(i.e. trunk hunting) obtained from Local Exchange Company tariffs. When an 800
Service Customer uses Vertical Features obtained by the Company from Local
Exchange Company tariffs, a charge will apply. This charge may not be counted
toward the attainment of any volume or revenue commitment and will not be
discounted.
3.4.13 In the event that a Customer cancels its 800 Service, the Customer may elect to
retain the Company as its Resp. argo .
3.4.14 In the event that a Customer cancels its Company Resp. Org. or 800 Service
the Customer shall be responsible for all outstanding indebtedness to the Company
and any outstanding charges applicable to any service obtained by or on behalf of the
Customer by the Company.

3.4.15 It is the Customer s responsibility to provide answer supervision back to the
Company point of connection even when the Company 800 Service is connected to
switching equipment or a Customer-provided communications system. In such case
the equipment or system must provide appropriate supervision so that the measure of
chargeable time begins upon delivery of the call to the Customer s switching
equipment or communications system and ends upon tennination of the call.
3.4.16 800 Feature Charges - Feature Charges are detennined by the specific feature
requested by an 800 Customer. These charges are in addition to 800 usage charges
and are not subject to discounting unless specifically indicated in Section 5, Service
Charges (5.4).
3.4.17 The Company Resp. Org. Charges - Where the Company serves as a Resp.
argo for a non-Company 800 Service Customer, the Company will pass on the tariffed
Local Exchange Carrier charges for SMS/800 Database and related services.

Reserved For Future Use
Promotional Offerings
Certain promotional offerings may be provided from time to time via this price list.
These promotional offerings may only apply to certain services, and may be limited to
certain dates, times, and locations. Inland Long Distance Company will provide
written notification to the Idaho Public Utilities Commission of any promotional
offerings.
