On October 10, 1996, Birdelle Brown sent a Memorandum to Myrna Walters re: U S West’s tariff advice proposing withdrawal of Business Telechoice from its Services Catalog. Birdelle’s Memorandum set out the particulars of the filing and the reason for the necessity of a Minute Entry.
Upon review of said Memorandum, we hereby approve Tariff Advice 96-02-S, this date - 10-15-96; effective 10-15-96, requested effective date of U S West.
