U S West Tariff Advice 95-03-S - REVISION
This Minute Entry is necessitated by an error discovered in U S West's Basic Local Exchange Tariff.  Details of the matter are set out in Jim Long's Memorandum to Myrna Walters, dated 9-14-95, copy of which is attached hereto.
Pages 31 and 35 of Section 5 of U S West's Basic Local Exchange Tariff have now been corrected.  They bear an effective date of 9-21-95.  Therefore, we hereby approved said pages this date - 9-21-95; effective 9-21-95.
