Smith, Joe Miller and Ralph Nelson and staff members Belinda Anderson, Joe Cusick, Bev Barker, Belinda Anderson, Marge Maxwell, Don Oliason and Myrna Walters.  Also in attendance was Pat Stewart of U S WEST. Items from the December 5, 1994 Decision Meeting Agenda were discussed and acted upon as follows. 1.  Regulated Carrier Division Agenda dated December 5, 1994. Approved. 2.  Belinda Anderson's November 25, 1994 Decision Memorandum re:  U S West (North) Tariff No. 94-11 Filed on November 22, 1994 to be effective December 23, 1994 that changes the Customer Acceptance Terms on Private Line Transport. Approved. 3.  Belinda Anderson's November 30, 1994 Decision Memorandum re:  U S West (South) Tariff No. 94-09-S Filed on November 23, to be effective December 26, 1994 That Allows Inmates to Make Local No-Charge Calls to Legal Counsel. Approved. 4.  Belinda Anderson's November 30, 1994 Decision Memorandum re:  U S West (North) tariff No. 94-12-N Filed on November 23, to be Effective December 31, 1994 That Expands 10XXX To 101 XXXX. Approved. 5.  Birdelle Brown's November 30, 1994 Decision Memorandum re:  EAS Petition for Toll-Free Service Between Weiser and Payette. Commissioner Smith had a question on this item.  Asked Birdelle Brown what it mean to the staff if the Commissioners include it in the GNR generic telephone case? Birdelle responded.  Some cases have kept their own case numbers while recently-filed petitions have been assigned the generic number until a decision was made on how to handle them. Decision was to do whatever was necessary for staff to keep track of the filings but that not all could be handled together. 6.  Washington Water Power Company; 3% Natural Gas Franchise Tax by the City of Hayden Lake, Idaho. Approved.   7.  Marge Maxwell's December 1, 1994 Decision Memorandum re:  Sargent Water Company 1994-95 Tariff to Recover DEQ's Assessment to Fund its Public Drinking Water Program. Commissioner Smith asked which charge Sargent Water Company would be charging for 94/95? Marge responded - the 75 cents per month. Approved. **Approved this date 12-5-94 - to be effective 12-6-94. 8.  Marge Maxwell's December 2, 1994 Decision Memorandum re:  Country Club Hills Utilities 1994-95 Tariff to Recover DEQ's Assessment to Fund its Public Drinking Water Program. Approved this date 12-5-94 to be effective 12-6-94. 9.  Marge Maxwell's December 2, 1994 Decision Memorandum re:  Warm Springs Mesa Water Company 1994-95 Tariff to Recover DEQ's Assessment to Fund its Public Drinking Water Program. Approved this date 12-5-94 to be effective 12-6-94. 10. Marge Maxwell's December 2, 1994 Decision Memorandum re:  Future 1994-95 Tariff Filing of Small Water Utilities For Recovery of DEQ's Assessment to Fund its Public Drinking Water Program. Commissioners want to show an effective ate after each utility submits its tariff and wants each filing brought to a decision meeting. Meeting adjourned.
