U S WEST - Advice 94-04-S
On July 21, 1994, U S West filed Advice No. 94-04-S.  Purpose of the tariff filing was to revise the Caldwell Exchange Map associated with Basic Local Exchange Tariff to include territory described in Case No. USW-S-94-1.  By Order No. 25573 in Case No. USW-S-94-1, U S West's Certificate of Public Convenience and Necessity was amended to include certain unassigned territory in Canyon County.
The attached Exchange Map is therefore approved this date 8-2-94, to be effective 8-21-94.
