BACKGROUND
On November 06,2023,Frontier Communications Northwest LLC dba Ziply Fiber
(the "Company")notified the Commission that it had changed its name to Ziply Fiber Northwest
LLC dba ZiplyFiber.As a result,the Company requests that the Commission update the
Company name in all Commission records.
STAFF ANALYSIS
On July 15,20211,Frontier Communications Northwest LLC dba Ziply Fiber changed its
name to ZiplyFiber Northwest LLC dba Ziply Fiber.The Idaho Secretary of State acknowledged
the transaction and issued the Certificate of Amendment of Foreign Registration2
The Commission has jurisdiction over the issuance of Certificates of Public Convenience
and Necessity ("CPCN")in Idaho.IDAPA 31.01.01.114.The Company holds a CPCN
Certificate No.383 that was issued pursuant to Commission Order No.32142 in Case No.VZN-
T-00-01.Additionally,the Commission has jurisdiction over the granting of the Eligible
Telecommunications Carrier ("ETC")designation status.47 U.S.Code §214(e)(l)(2).The

Company was granted the Eligible Telecommunications Carrier ("ETC")Designation Status
pursuant to Commission Order No.27245 in case No.GTE-T-97-12.
Staff examined the request and determined that this action will simply amend the name
on the CPCN Certificate and on the ETC designation status to reflect the recent corporate name
change of the Company.
STAFF RECOMMENDATION
Staff recommends that the Commission approve this request.
