EXTREME MEDIA TECHNOLOGIES, INC., TO CLEAR VOICE
TELECOM, INc. CASE NO. EXT - T 05-01.
BACKGROUND AND DISCUSSION
On October 17, 2008, Clear Voice provided a letter to the Commission indicating that it
had changed its name from Extreme Media Technologies, Inc., to Clear Voice Telecom, Inc.
Extreme Media was issued Certificate No. 455 on March 10 2006 in Case No. EXT-05-01.
Staff contacted the Company and explained that in order to change the Company s certificated
name it would have to change the name that is registered with the Secretary of State as well as
file a revised price list with all the updated company information.
For the past year the Company has gone through a number of management and
organizational changes. In December, 2008 , the Company hired legal counsel who has been
working with the Commission Staff to resolve the outstanding issues needed to change the
company name and bring the price list into compliance with the Commission s rules.
On September 17,2009, the Company filed its final Application to change its certificated
name along with its updated Secretary of State authorization and a complete replacement price
list reflecting the new organization s information.

STAFF RECOMMENDATION AND COMMISSION DECISION
Staff has reviewed the Company s filing and finds that the changes now comport with the
Idaho Code and Commission s rules. The new price list also comports with the Commission
customer relation rules. With all the changes and revision in place Staff now recommends
approval.
