On March 3 2005, the Commission received Potlatch Telephone Co. Advice Letter No.
OS-OIP. In this filing, Potlatch requests the tariff Title Page be modified to include dba TDS
Telecom. After Staff discussion with the Potlatch Tariff Administrator, the Commission
received an amended filing on March 30 2005. In the revised filing, Potlatch includes an
additional request to amend Certificate No. 263 to show TDS Telecom assumes the certificate.
ST AFF REVIEW
Staff has reviewed Advice Letter No. OS-OIP, the March 30 2005 amendment, as well as
the Certificate of Assumed Business Name from the Secretary of State s office and has no
objection. Staff recommends approval of the tariff filing and the modification of Certificate No.
263 to include the assumed business name ofTDS Telecom by adding .. . aka TDS Telecom" to
the Certificate.
