RE:VERIZON NORTHWEST, INC ADVICE NO. 04-06 REGARDING
APPLICATION OF THE FEDERAL SUBSCRIBER LINE CHARGE
On September 3, 2004, Verizon Northwest, Inc. submitted Advice No. 04-06. In this
filing, Verizon seeks to add an informational only footnote to the CyberDS 1 Service stating that
a credit of 100% of the applicable interstate Subscriber Line Charge will be applied to 22 of the
24 channels. Verizon requests an effective date of October 16, 2004.
STAFF REVIEW
Staff has reviewed the filing and, although the SLC is a federal charge, allows its
placement in Verizon' s local exchange tariff for informational purposes. Therefore, Staff
recommends approval of Advice No. 04-06.
