AND ITS) CASE  NO.  UWI-W-96-6 ABILITY TO PROVIDE ADEQUATE SERVICE)      AND WATER QUALITY.)    SECOND PRODUCTION )REQUEST OF THE )COMMISSION STAFF )TO UNITED WATER )IDAHO INC. _____________________________________________) The Staff of the Idaho Public Utilities Commission by and through its attorney of record, Scott Woodbury, Deputy Attorney General, requests United Water Idaho Inc. (UWI) provide the following documents and information, pursuant to Commission Rule of Procedure 225, IDAPA 31.01.01.225, on or before FRIDAY, JANUARY 17, 1997. This production request is to be considered as continuing, and United Water Idaho Inc. is requested to provide by way of supplementary responses, additional documents that you or any person acting on your behalf may later obtain that will augment the documents produced. Please provide answers to each question; supporting work papers that provide detail or are the source of information used in calculations; the name and telephone number of the person preparing the documents; and the name, location and telephone number of the record holder. REQUEST NO. 12:  United Water's response to Staff's Production Request No. 4 is not conclusive.  It appears from the information furnished that United Water is not in compliance with Utility Customer Relations Rule No. 401 (Complaint to Utility) and Rule 403 (Records of Complaint).  (See attached copy).  Please respond with further information regarding how a complaint is handled with the Customer Rules in place. REQUEST NO. 13:  Please update United Water’s response to Staff Production Request No. 3 to include complaints to the end of 1996 calendar year.  In addition, if customer account numbers are available for complaints detailed in the previous response, please provide them.  Finally, if the information that is combined into single columns (i.e. month, day, year into date; first and last names; and street numbers and streets) can be separated into individual columns as originally requested, please do so.
