The Staff of the Idaho Public Utilities Commission, by and through its attorney of record,
Neil Price, Deputy Attorney General, requests that Spirit Lake East Water Company (Company;
Spirit Lake) provide the following documents and information on or before THURSDAY,
MAY 9, 2013.
This Production Request is to be considered as continuing, and Spirit Lake East Water
Company is requested to provide, by way of supplementary responses, additional documents that it
or any person acting on its behalf may later obtain that will augment the documents produced.
For each item, please indicate the name of the person(s) preparing the answers, along with
the job title of such person(s) and the witness who can sponsor the answer at hearing.
SECOND PRODUCTION REQUEST 1 APRIL 18, 2013
TO SPIRIT LAKE EAST
REQUEST NO. 25: Please provide the Company's written record of complaints and
requests for conference for calendar years 2010, 2011, 2012 and 2013 to date.
REQUEST NO. 26: Does the Company request deposits from customers? If so, please
provide an explanation of how the Company calculates the amount of deposit and a sample copy
of a deposit request letter.
REQUEST NO. 27: Please provide a sample copy of a bill sent to customers (a) with no
past due balance and (b) with a past due balance. Please provide a sample copy of the proposed
monthly billing statement, if different from the current billing format.
REQUEST NO. 28: Please explain how bills will be calculated under the proposed
change to monthly billing for the month(s) when the Company is unable to read the meters and
subsequent months when the Company is again able to read meters.
REQUEST NO. 29: Does the Company offer customers a pre-printed form for the
medical emergency certificate? If so, please provide a copy of the form.
REQUEST NO. 30: Please provide a sample copy of the Summary of Rules provided to
customers. Please provide an explanation of how and when it is sent?
REQUEST NO. 31: Please provide a copy of the explanation of rate schedules provided
to customers. Please provide an explanation as to how and when it is sent.
REQUEST NO. 32: Please provide an explanation of how the Company handles its past
due bills (collection process).
REQUEST NO. 33: Please provide copies of the following Company documents: Initial
Notice to Terminate, Final Notice of Intent to Terminate Service, and the Notice of Procedure for
Reconnecting Service left at the premises following disconnection of service.
