CASE NO. SPL-04- FIRST PRODUCTION REQUEST OF THE COMMISSION STAFF TO SPIRIT LAKE EAST WATER COMPANY The Staff of the Idaho Public Utilities Commission, by and through its attorney of record, John R. Hammond, Deputy Attorney General, requests that Spirit Lake East Water Company (Spirit Lake; Company) provide the following documents and information on or before WEDNESDAY, FEBRUARY 25, 2004. IDAPA 31.01.01.225 and .229. The Company is reminded that responses to production requests must include the name and phone number of the person preparing the document, and the name, location and phone number of the record holder. IDAPA 31.01.01.228. This Production Request is to be considered as continuing, and Spirit Lake is requested to provide, by way of supplementary responses, additional documents that it or any person acting on its behalf may later obtain that will augment the documents produced. FIRST PRODUCTION REQUEST TO SPIRIT LAKE EAST WATER COMPANY JANUARY 27 , 2004 Please provide answers to each question; supporting workpapers that provide detail or are the source of information used in calculations; the name and telephone number of the person preparing the documents; and the name, location and telephone number of the record holder. For each item, please indicate the name of the person(s) preparing the answers, along with the job title of such person(s) and the employee who can sponsor the answer at a public meeting. For all responses to the following requests, please provide all workpapers and include an electronic version on diskettes (3.5 in.) and all underlying formulas in Excel (version 5) language. Request No.1: Please provide a complete system map showing: location of all existing and proposed facilities, the service area boundary, the lots within the service area, those lots now taking service and those lots where service may be required. Request No.2: Please provide a detailed description of the typical installation required for a new hook-up. Request No.3: Please provide a detailed cost estimate for all items included in a typical installation. Request No.4: Does the Company use a contractor to perform any part of the hook-up installation or are the efforts performed with Company personnel? Request No.5: If any hook-up installations, or parts there of, are performed by a Contractor: Describe the work performed by the Contractor. Does the Company solicit bids for the hook-up installations? How often are bids received? Please provide the most recent bid summary (or other document) that includes the rates charged by the Contractor for equipment hours, labor hours, mobilization, and other costs associated with hook-up installations. Request No.6: The Company is requesting a 20% contingency be included in the hook- up fee. Please explain why a contingency is necessary and how the 20% was calculated. Request No.7: Does the system have sufficient water supply to provide adequate service to all lots in the service area? Please provide the Company s available supply capacity and average customer usage information. FIRST PRODUCTION REQUEST TO SPIRIT LAKE EAST WATER COMPANY JANUARY 27 2004 Request No.8: How will the Company determine whether a hook-up fee is charged or not? Request No.9: Will the hook-up fee be charged only once per lot? Ifmore than once per lot please explain. Request No. 10: Please provide an accounting of the Company s annual labor/vehicle expenses over the last 3 years. Please identify these costs for each year, the number of labor and vehicle hours incurred each year, the rate charged for labor and vehicle hours each year, and provide copies ofthe associated timecards and vehicle logs. Request No. 11: Please provide the Company s method of allocating labor/vehicle expenses. Indicate how the annual labor/vehicle expenses are allocated between operations hook-ups and any other activities. Request No. 12: Please provide an accounting ofthe Company s annual administrative costs over the last 3 years. Please identify these costs for each year, the number of administrative hours incurred each year, the rate charged for those hours each year, and provide copies of the associated timecards. Request No. 13: Please provide the Company s method of allocating administrative costs. Indicate how the annual administrative costs are allocated between operations, hook-ups and any other activities. Request No. 14: Please provide the number of hook-ups the Company has had in each of the last 5 years. Request No. 15: What is the Company s estimated number of hook-ups per year for the foreseeable future? Request No. 16: Please provide a copy of each hook-up invoice for 2002 and 2003.
