ROC-W-08-1 ) ) ) FIRST PRODUCTION REQUEST ) OF THE COMMISSION STAFF ) TOROCKYMOUNTAIN ) UTILITY COMPANY, INC. ) The Staff of the Idaho Public Utilties Commission, by and through its attorney of record, Kristine A. Sasser, Deputy Attorney General, requests that Rocky Mountain Utility Company, Inc. (Company; Rocky Mountain) provide the following documents and information on or before FRIDAY, AUGUST 8, 2008. This Production Request is to be considered as continuing, and Rocky Mountain is requested to provide, by way of supplementary responses, additional documents that it or any person acting on its behalf may later obtain that will augment the documents produced. Please provide answers to each question, supporting workpapers that provide detail or are the source of information used in calculations. The Company is reminded that responses pursuant to Commission Rules of Procedure must include the name and phone number of the FIRST PRODUCTION REQUEST TO ROCKY MOUNTAIN UTILITY COMPANY 1 JULY 25, 2008 person preparing the document, and the name, location and phone number of the record holder and if different the witness who can sponsor the answer at hearing if need be. Reference IDAPA 31.01.01.228. In addition to the written copies provided as response to the questions, please provide all Excel and electronic fies on CD with formulas activated. REQUEST NO.1: Staff notes that Pepperwood Crossing Subdivision is approximately 4.5 miles from the Rigby city center. Please indicate how close Rigby's water distribution system is to the Pepperwood Crossing's water distribution system. REQUEST NO.2: When did the Company star delivering water to the first residential customer and star charging water fees? REQUEST NO.3: Staff notes that JeffIreland is the listed certified operator for the systems. Please provide a copy of any and all service contracts detailng his responsibilties, contract amount and curent contact information. REQUEST NO.4: When was the water system serving Pepperwood Crossing Subdivision (Phases 1 and 2) completed? Please provide a copy of the approval letter from the Idaho Department of Environmental Quality indicating final approval of the construction of the water system and authorization to star serving water to customers. REQUEST NO.5: Please provide a copy of the following engineering reports: a) Demonstration of Financial, Technical, and Managerial Capacity for a New Public Water System - Pepperwood Crossing b) Operations Manual, and c) As-built Drawings. REQUEST NO.6: Please provide a plat map of service area showing the location of all waterlines, wells (l & 2), pumps, fire hydrants, gate valves, customer shut-off valves, and other key appurenances of the water supply and distribution system serving Pepperwood Crossing Subdivision (Phases 1 and 2), if not included in Request NO.5 above. FIRST PRODUCTION REQUEST TO ROCKY MOUNTAIN UTILITY COMPANY 2 JULY 25,2008 REQUEST NO.7: What are the sizes (diameter in inches) of service lines for residential and commercial customers, if not included in Request NO.6 above? REQUEST NO.8: Please provide any engineering, groundwater hydrology or other reports or studies that were used for determining the location and depth of the ground water source of domestic water supply. REQUEST NO.9: Please provide any engineering report or calculation that has determined the actual number and class of customers that can be served by the water system in Pepperwood Crossing Subdivision (Phases 1 and 2). REQUEST NO. 10: Staff notes that the primar well pump is metered. Please provide monthly production well flow data since Rocky Mountain Utilty staed delivering water to customers up to the most recent month (ending in June 30, 2008), expressed in gallons. Please include the number of customers (all classes-residential and commercial) during those months. REQUEST NO. 11: Staff notes that there are eight potential commercial customers at the Pepperwood Crossing Subdivision. The Company is proposing a monthly rate of $42.00 per customer. Does this include commercial as well as residential customers? REQUEST NO. 12: When the Application for CPCN was fied in May 2008, there were 38 residential customers and no commercial customers connected to the system. Please provide an updated number to date. REQUEST NO. 13: A Sanitar Surey conducted by IDEQ on September 28,2007, indicated that the Pepperwood Crossing water system was in violation of the Idaho Rules for Public Water Systems promulgated by IDEQ which requires a minimum of two (2) sources for new community water systems served by ground water and intended to serve more than 25 homes. Please provide the status of driling the additional well and connecting it to the current FIRST PRODUCTION REQUEST TO ROCKY MOUNTAIN UTILITY COMPANY 3 JULY 25, 2008 distribution system of Pepperwood Crossing. Please provide the cost incured to date and the estimated costs of completing the project. REQUEST NO. 14: Staff notes that there is a valid water permit for the primar well curently being used. Please provide the status of the water permit for the second well to be developed or curently being developed. REQUEST NO. 15: Staf notes that there was a transfer of shares from the Harson Canal and Irrigation Company to Jim Bernard, owner of Rocky Mountain Utility. Please explain the significance of this transfer to the Company. Is there a plan to use the irrigation water rights? Please explain. REQUEST NO. 16: Please provide your plan for connecting new customers and the proposed hook-up fee. Provide justification for the amount proposed. REQUEST NO. 17: Please provide copies of your Billng Statements, Final Disconnect Notice, Notice of Procedure for Reconnecting Service, and Rules Sumar for review. REQUEST NO. 18: Please provide copies of the notice mailed to the customers for the Public Workshop and a copy of the Press Release. REQUEST NO. 19: Please provide explanation of how the Company conducts its termination process.
