The Staff of the Idaho Public Utilities Commission, by and through its attorney of record, Scott Woodbury, Deputy Attorney General, requests that Morning View Water Company Inc. (Morning View) provide the following documents and information on or before WEDNESDAY, SEPTEMBER 3, 1997. This Production Request is to be considered as continuing, and Morning View Water Company Inc. is requested to provide, by way of supplementary responses, additional documents that it or any person acting on its behalf may later obtain that will augment the documents produced. Please provide answers to each question; supporting workpapers that provide detail or are the source of information used in calculations; the name and telephone number of the person preparing the documents; and the name, location and telephone number of the record holder. Request No. 1:Attachment A to this production request is a list of items identified by Staff during its review of the Company’s bank statements for the years 1990 through May 1997.  For each item identified on the attachment please provide a detailed explanation of each expenditure and deposit to the bank account. Request No. 2:For each of the last 6 years please provide the number of customers connected and being served by the Company at year end. Request No. 3:How many customers have paid a connection or hook-up charge to the Company?  How much did each one pay? Request No. 4:What is the basis for charging the customers to connect to the system? Request No. 5:Who installs the water line from the Company’s curb stop to the customers home? Request No. 6:Who pays for the installation of the customers water line? Request No. 7:What other business activities is Morning View Water Company involved in other than the provision of domestic water service? Request No. 8:When the Company installs meters to existing customers, who will pay the cost of installation?  Will the customer be billed for this cost? Request No. 9:What is the official name of the entity responsible for building the water system? Request No. 10:Is the entity named in the answer to Request No. 9 above related in any way to Morning View Water Co.?  If so, how? Request No. 11:Please provide a copy of customer notice of request for rate increase.  Please provide date notice was mailed.   (Utility Customer Information Rule, IDAPA 31.21.02.01, Rule 102) Request No. 12:Please provide a copy of the Company’s Summary of Customer Rules.
