Dear Mr. Benton:
The DEQ has not received the Letter of Declaration form stating the number of service connections in the
Valley View Subdivision. The form is necessary in determining the classification of the Valley View water
system, and what, if any, monitoring will be required of the system.
The form was requested from you twice previously via written correspondence, including a certified letter
dated August 12, 2008, as well as verbal requests during two visits to our office. The DEQ once more
requests you fill out and return the attached Letter of Declaration form.
