The Staff of the Idaho Public Utilities Commission, by and through its attorney of record, Susan Hamlin, Deputy Attorney General, requests that Valley View Subdivision, Inc. provide the following documents and information, pursuant to Rule 225 of the Idaho Public Utilities Commission’s Rules of Procedure, IDAPA 31.01.01, on or before AUGUST 27, 1996.
This Production Request is to be considered as continuing, and Valley View Subdivision, Inc. is requested to provide, by way of supplementary responses, additional documents that it or any person acting on its behalf may later obtain that will augment the documents produced.
For each item, please indicate the name of the person(s) preparing the answers, along with the job title of such person(s) and the witness who can sponsor the answer at hearing.
Request No. 1: List the name and address of each customer currently using Valley View water service.
Request No. 2: How often are customers billed for water service?
Request No. 3: What are the current rates charged for water service for residential customers? For commercial customers? For other customers? If other customers, please list type of customer.
Request No. 4: What additional charges related to water service do you assess either on a recurring or nonrecurring basis, i.e., late payment fee, hook-up fee, customer charge, etc.?
Request No. 5: Please provide a copy of the following documents: customer bill; disconnection or late payment notice; water agreement; and any other documents routinely furnished to customers receiving water.
Request No. 6: Please provide copies of customer billing records which show amount of water used and the dollar amount billed for each customer for the period of August 1995 through July 1996.
