This Production Request is to be considered as continuing, and Packsaddle Development Corporation is requested to provide, by way of supplementary responses, additional documents that it or any person acting on its behalf may later obtain that will augment the documents produced. For each item, please indicate the name of the person(s) preparing the answers, along with the job title of such person(s) and the witness who can sponsor the answer at hearing. Request No. 1:What is the official name(s) of the subdivision(s) to which you provide water? Request No. 2:Please describe the specific geographic area you serve.  Include at least the following: a.Map(s) and legal description, if possible. b.Map(s) or development plat(s) showing the location, size and type of water main pipe. Request No. 3:What is the official name of the water system you operate for the subdivision(s)? Request No. 4:Regarding each water system, provide the following information: a.The names, addresses, phone numbers and respective ownership percentages of the owners; b.The title and duty, if any, of each owner with respect to the water system. Request No. 5:Please provide the total number of lots in each subdivision including a breakdown of those that are occupied and those not yet occupied, as of February 1, 1995.   Request No. 6:To how many customers do you provide water?  How many are residential users?  Commercial users?  Other users? Request No. 7:On what basis of water usage are the customers billed? Request No. 8:If metered: a.How many service connections have meters? b.How often are meters read? Request No. 9:If customers are not metered, what measurement is used to determine the amount of water supplied to customers? Request No. 10:How often are customers billed? Request No.11:Please supply a sample copy for each of the following:  customer bill, disconnection or late payment notice, water agreement, any other documents routinely furnished to customers receiving water. Request No. 12:What are the current rates used for billing of water service for residential customers?  For commercial customers?  For other customers? Request No. 13:What additional charges related to water service do you assess, i.e., late payment fee, hook-up fee, etc.? Request No. 14:Please supply a customer list, including name, address and phone number of each customer. Request No. 15:Is the water system a sole proprietorship, corporation, co-operative or mutual non-profit organization? Request No. 16:If incorporated, please provide a copy of the water system’s articles of  incorporation and by-laws as filed with the Secretary of State’s office. Request No. 17:In what name do you file your income tax returns for the water system(s)? Request No. 18:Please provide a copy of the water system’s 1993 Federal and State Income Tax Returns.   Request No. 19:Please provide complete breakdowns (for water systems only) of plant and expenses that can be documented or reasonably estimated.    Request No. 20:Please provide the title and a description of any groups or boards that represent customers of the water system.  Who composes these groups or boards? Request No. 21:Does the water system have an on-site manager?  Who do customers contact in case of an emergency with the water system? Request No. 22:Please provide the following information on each well: a.Depth of well and the static water level. b.HP rating of pump motor. c.Pump capacity in gpm. Request No. 23:If the water system has a reservoir, please provide its capacity in gallons. Request No. 24:If the water system has a booster pump, please provide its motor rating in hp and pump capacity in gpm. Request No. 25:What are the minimum and maximum system operating pressures? Request No. 26:How is system pressure maintained (regulator or elevated storage)? Request No. 27:If service connections are not metered, are there accessible shut off valves?
