The Staffof the Idaho Public Utilities Commission, by and through its attorney of record,
Daphne Huang, Deputy Attorney General, requests that Diamond Bar Water Company
(Diamond tsar; Company) provide the tbllowing documents and intbrmation as soon as possible,
but no later than TUESDAY, APRIL 512016.
This Production Request is to be considered as continuing, and Diamond Bar Water
Company is requested to provide, by way of supplementary responses, additional documents that
it or any person acting on its behalf may later obtain that will augment the documents produced.
For each item, please indicate the name of the person(s) preparing the answers, along
with the job title of such person(s) and the witness who can sponsor the answer at hearing.
THIRD PRODUCTION REQUEST

REQUEST NO. 16: Please answer the following questions regarding irrigation meters:
a. Who installs the irrigation meters (original connection)?
b. Who maintains the irrigation meters?
c. Who tests to insure accurate readings?
RESPONSE: The original connection of Irrigation Meters was performed by Diamond
Bar Estates Water Co. Since the year of 2008 the Irrigation Meters in all their entirety
are maintained, installed, uninstalled and the accuracy of the readings are the
responsibility of the home owners.
REQUEST NO. 17: lnformation provided in the 2007 case (DIA-W-07-01) indicates
that irrigation meters are removed and reinstalled on an annual basis. How are these costs
accounted tbr? Who pertbrms this work? How do you insure water quality is maintained'/
RESPONSE: As stated in the Response to Request No 16 the Home Owners are
responsible for the annual install and uninstall of the irrigation meters. Diamond Bar Estates
Water Co requires from the home owlers who maintain a sprinkler system to send the Company
the annual backflow test results report from their sprinkler company.
REQUEST NO. 18: What is the per meter cost of this removal and reinstallation for
irrigation meters referenced in the previous question?
RESPONSE: The response to Request 16 and 17 makes Request No 18 non applicable,
Except every spring and fall the Water Master turns the water on to each irrigation meters. The
time to do this is approximately 3 hrs. The turning on and otf of irrigation meters is considered a
separate wage, however, it is not. The Water Master is paid through the monthly $475 he
recelves. .
REQUEST NO. 19: Please provide a diagram of typical I inch meter installation.
RESPONSE: See Exhibit 17

REQUEST NO. 2l: What is the current (initial) connection charge for the irrigation
meter? Is the Company proposing to change the charge?
tlJi,SPONStl: Diamond tsar Estates Water Co does not have an initial connection
charge for irrigation meters. The Company is no longer responsible for irrigation meters in all its
entirety and does not propose any change in the future.
REQIIEST NO. 22: How does the Company measure the production flow from Well
#2 (emergency well)? Where is the meter that measures this flow located? Please provide
pumping data for the emergency well by day for 2014.
RESPONSE: The Company has a flow meter located at Well #2 site. The flow meter
readings for well #2 arc done once a month. The meter is read whether or not Well #l is
attached. See Exhibit22for pumping data.
REQUEST NO.23: In the event that there is a transfer of the land surrounding Well #2,
is there a plan to insure that this well is available for system requirements? Please explain.
RESPONSE: There is no plan or written agreement and do not foresee one for the
future. Well #2 has been used in an "As Needed" Basis and is a private well on private property.
If there is a transfer of land surrounding Well #2the well will no longer be available to Diamond
Bar Estates Water Co.
REQUEST NO. 24: Please provide a list of all water quality tests for the water sources
(wells) and the tests within the distribution system as required by the Idaho Department of
Environmental Quality. For each type of test, indicate the frequency, total cost per test and the
prorated yearly (normalized) cost. (For example, Well #1, arsenic test, every 3 years, $20 per
test, $20 per year, and so on).
RESPONSE: See Exhibit 18 and 18a
REQUEST NO.25: Is a backflow prevention device installed for every customer, for
both residential and for irrigation meters (for those customers having irrigation meters)?

RESPONSE: Customers know the devices are required and The Company is to receive
the annual report from the company who tums on their sprinkler systems in the spring. The
customers without sprinkler system are to have an atrnospheric breaker on their outside hose bib.
REQUEST NO.26: Are all backflow devices checked and certified as operational on an
annual basis? If not, how many backflow meters are checked?
RESPONSE: The customers are to have backflow prevention devices tested annually
by a Certified Tester and the Company is to receive the report. The Customers without a
sprinkler system are to have an atmospheric breaker on their outside hose bib.
REQUEST NO.27: Please provide a copy of the engineering report (from firm hired by
the Company) on the pump transformers. (Refer to response to Production Request 6).
RESPONSE: See Exhibit 19
RITQUIIST N0.2E: lollowing up on Production Request No. 6, please provide the
dates that the back up well was turned on from 2008 to 2013.

REQUEST NO. 29: [n response to Production Request No. I l, the Company stated that
Diamond Bar Water was not charged for water provided by Mr. Turnipseed's private well. Were
there any other reasons for not charging the water company for use of the well? Please explain.
RESPONSE: No. The Company's first priority is to get water to the Customers as
quickly as possible. When the well pump of well#l goes down it is an average of 5 day process
to replace, flush and take the appropriate tests.
REQUEST NO.30: Following up on Production Request 14, please provide the amount
that the Company could call upon from private equity, (i.e. from the owner, Mr. Tumipseed) to
fund repairs or system improvements in the case of an emergency.
RESPONSE: The Company has the ability to go to a bank and get the amount needed
at the time of repairs or system improvements.

REQUEST NO. 31: In Case No. DIA-W-07-01, Order No. 30455, the Commission set
Net Plant in Service (Rate Base) at $16,801. In the Application, the Company states that Rate
Base is $40,398. Following up on Production Request No. l, please provide a schedule of
capital projects that make up the difference from the previous case. Please include invoices, as
well as the date equipment was placed in service and the associated depreciation schedule.

REQUEST NO.32: Production Request No. 2 and Audit Request No. 10 indicated
there are three electricity purchase entries each month. Please explain what each of the three
different power invoices are for (i.e., pump house, shop, etc).
RESPONSE: Kootenai Electric Coop charges the Company for Well #l and Well #2.
Avista charges the Company for natural gas which heats the pump house, Well #1, during the
winter months.
REQUEST NO.33: Please provide an as built diagram of the water system.
RESPONSE: See Exhibit2D and20a
RIT,QUEST N0.34: Please provide a schedule showing the number ot'new connection
by year from 2008 to 2015.
RESPONSE: From 2008 through 2015 there the Company has had two (2) new
connections. 20121(one), 2015 1 (one)

REQUEST NO. 35: Please provide sample bill statements sent to customers under the
following circumstances:
(a) Customer is current on payment and has no past due balance;
(b) Customer owes a past due amount;
(c) Customer is billed only the monthly minimum charge (winter bill); and
(d) Customer is billed for usage accumulated over the winter (bill issued after the first
reading is taken in the spring).

REQUEST NO.36: Please explain how commodity charges are calculated in situations
where previous monthly bills were estimated or meter readings missed.
RESPONSE: The Company does not do estimating and meter readings are only
missed during the winter months which in the spring the bill is calculated as follows:
Months no readings done: Nov, Dec, Jan, Feb, and Mar. Meter read April 1't. Winter gallon
allowance would be27500 gals. Example: TL winter usage 35000-27500 :7500.

RJTQUEST N0. 37: Please explain the steps taken by the Company to collect past due
bills prior to actual termination of service.
RBSPONSE: Ordinarily The Company does not experience long overdue accounts.
There are a few who will linger as long as 90 days then catch up with a payment. When an
Owner rents their home The Company usually experiences difficulty collecting payment. First
step is stamping billing statement with our Past Due Stamp for approximately 3 billing cycles. If
there is no response from customer a discontinuation notice is sent. If still no results a phone call
is made stating service will be discontinued within 48 hours and proceed to post a notice of cut
of time and date on the door of house. The Company does not use a door tag.
REQUEST NO. 38: Please provide a copy of the following Company documents:
(a) Initial Notice to Terminate;
(b) Final Notice of Intent to Terminate Service; and
(c) Notice of Procedure for Reconnecting Service (door tag or notice, if used).

Observation Report
Dear Bob:
From our conversations and historical data received, it is our understanding that the well pump
located at Diamond Bar Estates has experienced problems since 2002; having to be replaced
multiple times. Since November 2012,the motor has been replaced three times (Appendix C).
On August 27, 2015 AEI Engineering, lnc. made its initial site visit for the purpose of providing
condition assessment and evaluation of the well house's electrical system serving the well pump.
The scope of this evaluation included analysis of the utility service, surge protection devices,
grounding system and pump motors.
On September 1, 2015 AEI observed the existing 6" pump motor being removed from service
and exchanged with a new'8' 60 HP pump. After being removed, AEI observed the existing
pump motor to have "blueing"; evidence of overheating. See appendix A. This is the fourth time
the motor has been replaced since November 2012.
A complete electrical metering investigation was conducted from September 8, 2015 through
September 11,2015. AEl, KEC and United Crown Pump and Drilling were contributors and/or
witnesses of this metering investigation. See appendices D, E and F.
The following summarizes the existing electrical system:
1. The facility is fed by Kootenai Electric Cooperative (KEC). The utility service is comprised
of 3 single phase, 25KVA, 480V, pole mounted transformers in a corner grounded Delta
configuration. The existing electricalservice, a fused disconnect, is located inside the well
house and is rated 2004 at 480V, 3 phase. The electrical service distance from the utility
transformer is over 200 ft.
2. The fused disconnect distributes three phase 480 volt power to a gutter system. The
gutter system serves (1) 60 HP well pump motor combination starter, (1) 20 HP fire pump
motor combination starter, (2) 3 HP booster pump motor combination starters and (1)
load center via a disconnect and transformer. This gives the site a total motor load of 86
HP. All motors have across-the-line starters.
3. The well pump is monitored and protected by a Franklin Electric SubMonitor, a Square D
SDSA3650 surge protection device and Delta Lightning Anestor.

4. The grounding system is comprised of grounding electrode conductors, bonds to piping,
a bond to the well case and ground rods.
5. The well pump feeder conductors between the starter and well head were replaced with 2
AWG copper conductors on September 2,2015.
The following electrical issues were observed:
1. The starting voltage dip on the system is approximately 15 percent.
2. The only Surge Protection Device (SPD) unit installed is under sized and located in the
well pump branch circuit which makes it less effective for the service entrance.
3. The cunent utility transformers are under sized.
a. Per the pump manufacturefs instructions (Appendix B), the 60 HP pump needs a
75kVA transformer. Cunently, the 75kVA transformer at full load can produce 90
amps. When all pumps are running, the service transformer is forced to produce
roughly 115 amps; this loads the transformer lo 128o/o rated load.b. The transformer is in a comergrounded delta configuration.
4. The 60 HP well pump has a Full Voltage Non-Reversing (FVNR) or "across-the-line
starte/'.
a. Per the KEC electric service handbook, motors over 20 HP are required to be soft
started (Appendix G).
5. The overall grounding (bonding) system is segmented and uses multiple connections,
conduit, equipment supports to make an interconnected system. This method of bonding
meets the minimum code requirements, however, it does not provide adequate grounding
for transient events as each connection provides a source of substantial voltage
differential during a transient event (lightning or power system disturbance).
6. Per NEC 230.90(8), the existing service equipment is not acceptable for the existing
corner grounded Delta utility service. A breaker must be used to simultaneously open all
phases (Appendix H).
7. The voltage drop from the well pump starter panel to the motor terminals was calculated
to be approximately 11.8 volts (Appendix E). Therefore, when voltage at the starter panel
is measured to be 471.7 volts or less, the motor is not being provided its rated voltage of
460 volts. This is shown in Appendix F.
Recommend actions to improve system protection:
1. Replace all end connectors on grounding system. Due to the sites history of lightning
strikes, replacing all of the end connections of the grounding conductors will give the
electrical system a more solid grounding system. This will provide increased protection
from lighting and faults in the system.
2. lnstall a new SPD to protect the service entrance of the system in addition to the existing
monitoring and protection equipment currently in use. At a minimum, provide the following
SPD:

a. Service entrance protection (Square D, HWA-8OKA).
3. Provide a larger 112.5 l(/A, 480V1277, Wye connected service transformer. Preferably, a
pad mount type located next to the building.
4. lf the service is not upgraded, replace the existing service fused disconnect.
5. Replace the FVNR across-the-line starter serving the well pump with a solid state soft
starter.
a. The soft starter will not only be less stressful on the electrical system (reduced
inrush cunent) and pump motor but will also provide a higher level of protection to
the motor while running.

Please contact our office immediately if payment has already been made. lf service is
disconnected it will be restored only after the full amount due has been paid. A reconnection

SERVICE ADDRESS
The company will postpone termination of utility service to a residential customer for thirty days
from the date of the receipt of certificate by a licensed physician or public health official that
states:
A. You or a member of 'your family, or other permanent resident of the premises where service
is rendered is seriously ill or has a medical emergency or will become seriously ill or have a
medical emergency because of termination of service, and
B. Termination of utility service would seriously affect you, a member of your family, or resident
of the household.
This certificate must be in writing and show clearly the name of the person whose serious illness
or medical emergency would be adversely affected by termination, and the name, title, and
signature of the person giving notice of or certifying the serious illness or medical emergency.
Prior to the expiration of any medical postponement you must contact the company and make
arrangement for the payment of the past due amount. YOU MAY BE PERMITTED TO PAY THE
PAST DUE AMOUNT IN REASONABLE INSTALLMENTS.
You may direct complaints concerning billing or termination of service to the company or to the
ldaho Public Utilities Commission. Such complaints may be made in person, in writing, or by
telephone to the company. Complaints td the ldaho Commission should be directed to the ldaho
Public Utilities Commission Staff, PO Box 83720 Boise lD 83720-0074, and Telephone 1-800-
432-0369
lf a complaint is made, your utility service will not be terminated as a result of the dispute that is
the subject matter of the complaint while the complaint is being reviewed by the company or by
