The Staff of the Idaho Public Utilities Commission, by and through its attorney of record, Lori Mann, Deputy Attorney General, requests that Country Club Hills Water Company provide the following documents and information on or before MARCH 17, 1995.
This Production Request is to be considered as continuing, and Country Club Hills Water Company is requested to provide, by way of supplementary responses, additional documents that it or any person acting on its behalf may later obtain that will augment the documents produced.
For each item, please indicate the name of the person(s) preparing the answers, along with the job title of such person(s) and the witness who can sponsor the answer at hearing.
Request No. 1:Please provide a copy of the notice the Company sent to its utility customers announcing the change of status and the intention to deregulate. Include the number of complaints and/or inquiries received in response.
Request No. 2:Please submit your annual PUC report for the year 1994. The Commission Staff will consider your request for decertification before the normal April 15 deadline for annual reports if your Annual Report is received before that date.
Request No. 3:Please provide the title and a description of any groups or boards that represent customers of the water system. Who composes these groups or boards?
Request No. 4:Please describe the degree of customer participation in the affairs of the Company, including the right to vote on seating or removal of directors, rate changes, and customer safeguards presently provided by IPUC regulation.
Request No. 5:Provide a copy of Form 1023, 1024 or 1028 Application for Recognition of Exemption submitted to the Internal Revenue Service under Section 501(c) of the Internal Revenue Code of 1986. If exemption has been granted, please submit a copy of the IRS approval documents.
Request No. 6:Please provide a copy of the water system’s 1993 Federal and State Income Tax Returns.
