CCH-W-12-01 FIRST PRODUCTION REQUEST OF THE COMMISSION STAFF TO COUNTRY CLUB HILLS UTILITIES The Staff of the Idaho Public Utilities Commission, by and through its attorney of record, Neil Price, Deputy Attorney General, requests that Country Club Hills Utilities (Company; Country Club Hills) provide the following documents and information as soon as possible, but no later than THURSDAY, MAY 31, 2012. The Company is reminded that responses pursuant to Commission Rules of Procedure must include the name and phone number of the person preparing the document, and the name, location and phone number of the record holder and if different the witness who can sponsor the answer at hearing if need be. Reference IDAPA 31.01.01.228. This Production Request is to be considered as continuing, and Country Club Hills is requested to provide, by way of supplementary responses, additional documents that it or any person acting on its behalf may later obtain that will augment the documents produced. FIRST PRODUCTION REQUEST TO COUNTRY CLUB HILLS MAY 10, 2012 Please provide answers to each question, supporting workpapers that provide detail or are the source of information used in calculations, and the name, job title and telephone number of the person preparing the documents. Please identify the name, job title, location and telephone number of the record holder. Please provide all Excel and electronic files on CD with formulas activated. REQUEST NO. 1: Please provide a copy of a map showing the general layout of Country Club Hills Water System including wells and pumps, main/distribution systems and general area/lots served by the Company. REQUEST NO. 2: The Company indicated in the Application that it provides water service to 147 residential customers and one commercial customer. Staff understands that all water services rendered to customers are metered with the exception of 10 residential customers in two condominiums. Please provide the various meter sizes installed by the Company and the breakdown of the meter size for all customer classes receiving service. For example, 100 residential customers - 1/4 inch meter, etc. REQUEST NO. 3: How many undeveloped lots still remain in the Company's service area that could potentially request water service in the future? How many new connections (residential and commercial) do you anticipate in the next two to three years? What service line/meter size do you expect to install for new service connections (residential and commercial)? REQUEST NO. 4: Staff understands that once every five years IDEQ conducts a Sanitary Survey on public water systems it regulates. When did IDEQ complete its Sanitary Survey for Country Club Hills' water system? Please provide a copy of the Sanitary Survey Report conducted by IDEQ. If there were deficiencies cited in the report, please provide the status of those deficiencies. FIRST PRODUCTION REQUEST TO COUNTRY CLUB HILLS 2 MAY 10, 2012 REQUEST NO. 5: Please provide any water quality and water system issues or violations issued by the IDEQ since the Company's last general rate case and the status of compliance of those issues/violations, if there are any. REQUEST NO. 6: Please provide any written records of complaints, requests for conferences and resolutions of issues specifically related to water system operation and water quality such as low system pressure, sand in water delivered to customers, system leaks, etc., since the Company's last general rate case. REQUEST NO. 7: Please provide monthly water production data and appropriate yearly totals for calendar years 2009, 2010 and 2011. REQUEST NO. 8: Please provide monthly water consumption data for each class of metered customers served (residential, commercial, landscaping customers) and the appropriate total for calendar years 2009, 2010, and 2011. Please provide supporting data used in the calculations (i.e., billing information showing volume of water sold for each customer by month in electronic format-Excel File). REQUEST NO. 9: Please provide an estimate of total unaccounted water losses due to leaks, wasted water, etc. by month for calendar years 2009, 2010, and 2012. Please explain how these losses are estimated. REQUEST NO. 10: The Company is proposing to increase the flat rate residential/commercial rates from $17 per month to $25 per month. Application, page 1. This rate schedule does not currently exist in the Company's tariff as approved by the Commission during the Company's latest rate case (Commission Order No. 29794, Case No. CCH-W-05-1). Is the Company proposing to add this new rate schedule to its tariff? If the answer is yes, please provide justification for this proposal. Please state how many customers are covered by this proposed tariff. FIRST PRODUCTION REQUEST TO COUNTRY CLUB HILLS 3 MAY 10, 2012 REQUEST NO. 11: For the monthly metered residential/commercial rates, the Company is proposing to change its rate structure from uniform rate with 30,000 gallons minimum charge volume allowance to an inverted (increasing) block rate design (2 blocks) with minimum charge volume allowance of 15,000 gallons. Please respond to the following: a)State the Company's rationale in proposing to change the rate structure from uniform rate design to inverted block rate designs; b)State the Company's rationale in decreasing the minimum (base) charge volume allowance from 30,000 gallons to 15,000 gallons (basis for selecting 15,000 gallons); and c)State the Company's rationale in selecting the first block (15,000-25,000 gallons) and the tail block (2 nd block over 25,000 gallons) in its proposed rate design. REQUEST NO. 12: Please state the Company's rationale for proposing a 51% increase ($15.75 to $23.75) per month for Flat Rate Condo rates compared to the Company's proposed general rate increase of 32%. REQUEST NO. 13: Please provide the Company's rationale for proposing an 18.8% increase (from $20 to $23.75) in the base monthly charge for metered landscaping rate as compared to a 47% increase (from $17 to $25) in the base monthly charge for metered residential and commercial customers. REQUEST NO. 14: Please provide documentation showing the Company's proposed rates will meet the Company's proposed revenue requirement (rate proof). REQUEST NO. 15: Please provide the number of customers for each customer class at the beginning and end of the year for calendar years 2009, 2010 and 2011. REQUEST NO. 16: Please provide a list of all water quality tests for the two water sources (wells) and the tests within the distribution system as required by the Idaho Department of Environmental Quality. For each type of test, indicate the frequency, total cost per test and FIRST PRODUCTION REQUEST TO COUNTRY CLUB HILLS 4 MAY 10, 2012 the prorated yearly (normalized) cost. (For example: Well No. 1, arsenic test, every 3 years, $20 per test, $6.67 per year; nitrate test, yearly, $20 per test, $20 per year, etc.) REQUEST NO. 17: In reference to the Company's proposal to increase the hook-up fee from $500 to $750 per installation. Please respond to the following: • Provide a copy of typical plan or sketch of new hook-up installation; • Itemized list of materials and components, unit cost per component and total cost of materials/components per hook-up; . Itemized labor costs (hours and rate) per installation; and • Explain if new hook up is completed by Company personnel or done by outside contractors. REQUEST NO. 18: Please provide a list of various capital improvement projects initiated and completed since the last general rate case. Briefly describe the total cost incurred, completion date and justification for each project. REQUEST NO. 19: Please provide an explanation and documentation (competitive bid documents, selection process, work contracts, etc.) showing that cost control efforts have been applied in paying any of the work elements for all the projects noted in Request No. 18 and demonstrating that those projects were built as economically as possible. REQUEST NO. 20: Please provide the name of the licensed operator for Country Club Hills. Please provide proof that the operator's license is current.
