This Production Request is to be considered as continuing,and the Company is requested to provide,by way of supplementary responses,additional documents that it,or any person acting on its behalf,may later obtain that will augment the documents or information produced. Please provide answers to each question,supporting workpapers that provide detail or are the source of information used in calculations,and the name,job title,and telephonenumber of the person preparing the documents.Please also identify the name,job title,location,and telephone number of the record holder. SECOND PRODUCTION REQUEST TO ALGOMA WATER 1 MAY 17,2023 In addition to the written copies provided as response to the requests,please provide all Excel spreadsheets and electronic files with formulas intact and enabled. REQUESTNO.2:Please provide a complete description of the Company's current operations and processes,including: a.Manpower/staffing,includingthe amount of labor and the level of expertise/experience; b.Policies and standards (i.e.,service level,maintenance turnaround time,etc.); c.Materials and supplies (productive material used for water quality,spare parts inventory,etc.);and d.Processes involved in the delivery of water and business processes (i.e.,billing, maintenance,customer support,etc.). REQUESTNO.3:Please separately describe any deficiencies needed to address system reliability,system integrity,and/or to support potential future growth: a.Identify any changes or investments the Company plans to implement to address each individual deficiency;and b.Please include the costs and identified benefits for any investments or increase in expenses. REQUESTNO.4:Please provide monthlyflow rate data,includinggallons pumped, for each system well pump and each booster pump during the past three years. REQUESTNO.5:Please provide monthlyelectric bills for each system well pump and each booster pump during the past three years.These bills would show monthlyenergy use (kWh)and peak energy demand (kW). REQUESTNO.6:Please provide the followinginformation: a.The make,model,and rating of each well pump; b.The make,model,and rating of each well pump motor; SECOND PRODUCTIONREQUEST TO ALGOMA WATER 2 MAY 17,2023 c.The make,model,capacity,and limit switch settings of each pressurized storage tank; d.The make,model,capacity,and limit switch settings of each unpressurized storage tank/reservoir.(Note:Also include referenced elevations for normal, high,and low static water levels.); e.The make,model,and rating of each booster pump;and f.The make,model,and capacity of all water treatment equipment. REQUESTNO.7:If the system supplies fire prevention capacity,please provide the following: a.The name and contact information for the fire authority who prescribes requirements for the system;and b.Fire flow and storage requirements prescribed by the fire authority. REQUESTNO.8:Please provide information related to the capacity and sizing of the system and the ability of the system to meet historical and future system peak demand.This information would include not only capacity information,but also any constraints within the system to meet peak demand.Please also provide information on average consumption and consumption at peak periods. REQUESTNO.9:Water system operators are licensed by the Idaho Board of Drinking Water and Wastewater Professionals.Please provide the followinginformation about your water system operator: a.Name; b.Operator license number and class;and c.Business address. REQUESTNO.10:For each water right used by the system,please provide the Idaho Department of Water Resources ("IDWR")Water Right Numbers (basin and sequence number). SECOND PRODUCTION REQUEST TO ALGOMA WATER 3 MAY 17,2023 REQUESTNO.11:For each well used by the water system,please provide the following: a.The IDWR Well Tag Number (D-Tag Number)currentlyon-file with IDWR; b.The location (Township,Range,Section,and Tract);and c.The well log. REQUESTNO.12:If Algoma Water System uses QuickBooks,please provide a copy of the Company's QuickBooks file.Please ensure that this file has data from January 1,2019 through December 31,2022.Please also note which edition of QuickBooks is currentlyused, and provide sufficient credentials,such as username and password,to access the file.An external accountant login for the QuickBooks file should be sufficient. REQUESTNO.13:Please provide a printed copy or PDF file of the Company's income statement and balance sheet from the accounting and financial software used by the Company for the Calendar Years Ended ("CYE")2019 through 2022. REQUESTNO.14:Please provide all supporting documentation for revenues posted in the 2022 annual report,includingamounts billed,amounts collected and all copies of customer billings from January through February 2023. REQUESTNO.15:Please provide a customer list broken out by customer type. REQUESTNO.16:Please provide a spreadsheet detailing Algoma Water System Account 101 Plant in Service,or Assets,separately identified by each sub-account number or category,as of the fiscal year ended December 31,2022.Please include in your response the following: a.Plant in Service Account Number; b.Original Price of the Asset; c.Net Book Value of the Asset; d.Date Asset Placed in Service; e.Current Annual Depreciation Expense; SECOND PRODUCTION REQUEST TO ALGOMA WATER 4 MAY 17,2023 f.Accumulated Depreciation;and g.Any receipts or backup documentation for all Plant in Service purchases since the last rate case in 2007. REQUESTNO.17:Please provide a list of all Plant in Service expected to be added through year-end 2023 and,please provide the expected cost. REQUESTNO.18:Please provide supporting documentation,including invoices and contracts,for the followingexpenses for close-of-year-ending December 31,2022.Please include any allocation factors,if applicable,for these expenses: a.Operations and Maintenance 2022 Labor Expense; b.Exterior Tank Painting Contract and Final Payment; c.Purchased Power Expense; d.Chemicals; e.Materials &Supplies O&M;and f.Idaho Rural Water Fee. REQUESTNO.19:Please provide the expected labor expenses for 2023 not included in the installation of the main line extensionand water tower capital expenses.Include a cost breakdown by month. REQUESTNO.20:Please provide an estimate of total unaccounted water losses due to leaks,wasted water,etc.by month over the past 5 years.Please explain how these losses are estimated. REQUESTNO.21:Please describe the frequency and cost of water testing for each contaminant that is tested. REQUESTNO.22:Please provide all receipts from any purchases made for the water system in 2022. REQUESTNO.24:Please describe the Company's methodology for developing, managing,executing,and completing Capital Projects in a least cost manner.Please provide a copy of all Company policies and procedures documenting the process. REQUESTNO.25:Please provide a schedule of all investments and retirements to Plant in Service since July 2008,including installation date,account,installed cost,and estimated depreciable life.Please provide the followinginformation: a.A full breakdown of the costs associated with Power Pumping Equipment; b.A full breakdown of the costs associated with Transmission,and Distribution Mains and Accessories;and c.A full breakdown of the cost associated with Other Plant &Misc.Equipment. REQUESTNO.26:Please provide a geospatial map of the Company's system identifying where customers are located,and the location and elevation of major equipment including wells,distribution piping,storage tanks,booster pumps,shut-off valves,meters,fire hydrants,and water treatment equipment. a.Please provide a list of each piece of equipment cross-referenced to the equipment on the map.For each item on the list,please provide a complete description of each item (i.e.,length and size of piping;number and specifications of pumps, size of storage facilities,the general condition of the equipment,length of time in service,etc.). REQUESTNO.27:Does the Company expect customer growth?If so,please provide the potential number of customers and type of customers by year.
