AWS-W-07-1 SECOND PRODUCTION REQUEST OF THE COMMISSION STAFF TO ALGOMA WATER COMPANY The Staff of the Idaho Public Utilties Commission, by and through its attorney of record, Donald L. Howell, II, Deputy Attorney General, requests that Algoma Water Company (Company; Algoma) provide the following documents and information as soon as possible, but no later than THURSDAY, MARCH 20, 2008. The Company is reminded that responses pursuant to Commission Rules of Procedure must include the name and phone number of the person preparing the document, and the name, location and phone number of the record holder. Rule 228, IDAPA 31.01.01.228. This Production Request is to be considered as continuing, and Algoma is requested to provide, by way of supplementar responses, additional documents that it or any person acting on its behalf may later obtain that wil augment the documents produced. SECOND PRODUCTION REQUEST TO ALGOMA WATER 1 FEBRUARY 28,2008 Please provide answers to each question; supporting workpapers that provide detail or are the source of information used in calculations; the name and telephone number of the person preparng the documents; and the name, location and telephone number of the record holder. For each item, please indicate the name of the person(s) preparing the answers, along with the job title of such person(s) and the witness who can sponsor the answer at hearng. REQUEST NO. 23: The response to Request NO.2 in Staffs First Production Request indicated that water provided to customers is not treated in any way. Is the system chlorinated or otherwse disinfected? How do you disinfect the system? Howoften? Please explain briefly. REQUEST NO. 24: Attchment 3 to Request NO.5 in Staffs First Production Request, shows overall water production by the system. It is Staff s understanding that another meter was installed at the reservoir storage on the discharge side of the booster pumps. If flow records are made for this meter, please provide such data to reconcile and correlate readings from the well meter. REQUEST NO. 25: A copy of the letter (see attachment) from Mr. Robert Hansen, operator of the Algoma Water System, dated July 26, 2006 to Mr. Anthony Davis of the Idaho Deparment of Environmental Quality was provided to the IPUC. This letter discussed the status of the deficiency of Algoma Water Systems as a result of the Sanitar Surey conducted by IDEQ in 2005. What is the curent status of these deficiencies specifically, Items 2,3,4,5,6, and 7? REQUEST NO. 26: The response to Request No. 16 in Staffs First Production Request indicated that expensive repair work was done during the spring of 2007. Please explain these repair items for the water system and provide detailed cost data, including invoices. REQUEST NO. 27: IPUC has received comment from a customer indicating that the shut offvalve serving the customer's property could not be located by Algoma Water staff when the customer requested to be shown its location. Staff notes that the system is not individually metered and does not have meter boxes. Does the Company provide an individual shut off valve in the service line? How does the Company locate this valve where there is no valve box? SECOND PRODUCTION REQUEST TO ALGOMA WATER 2 FEBRUARY 28, 2008 REQUEST NO. 28: IPUC also has received comment from a customer indicating that the drain field for all the septic tans is just two lots away from the well and it containates the well regularly. Staff notes that test results done in 2006 reported to IDEQ indicate no violations for nitrate and fecal coliform, potential containation indicator from sources including leachate from septic tan and sewage or from human and anmal fecal wastes. Have there been any recent tests on these contaminants? How often does the Company test for microbial containants? Was the result of the latest test in compliance with IDEQ? Please provide a copy of the latest test results. REQUEST NO. 29: Please provide copies of the invoices totaling the $4,565.24 in Repairs and Maintenance expenses shown on the 2006 Profit and Loss statement and copies of invoices for all maintenance in 2007. REQUEST NO. 30: Please provide a copy of the 2007 Profit and Loss statement for Algoma Water Company. REQUEST NO. 31: Please provide a list of the capitalized assets of Algoma Water Company. REQUEST NO. 32: For each asset listed in response to Request No. 31, please provide ' the associated anual depreciation, the accumulated depreciation, and the date the asset was placed in service. REQUEST NO. 33: Please provide a chart of accounts and description of accounts used by Algoma Water Company. REQUEST NO. 34: Please provide copies of the 2006 and 2007 general ledgers. REQUEST NO. 35: Please provide a debt schedule showing all long-term and short- term amounts outstanding, stated interest rates, premiums, discounts, unamortized issuance expenses, and effective cost rates for 2007. Please see atached. 9. lOAP A 58.01.08.554.02 requires an operator who is certíìed by the Idao Burau of Occupation Licenses be responsible of the day to day opertion of the water system.
