Commission of a change in its operating entity that affects its Certificate of Public Convenience
and Necessity No. 492. On March 16, 2020, Velocity The greatest Phone Company Ever, Inc. filed
a Certificate of Amendment with the Idaho Secretary of State amending the entity name to
Velocity, A Managed Services Company, Inc.
IT IS HEREBY CERTIFIED that this Second Amended Certificate of Public
Convenience and Necessity, Certificate No. 492, is issued to Velocity, A Managed Services
Company, Inc., Certificate No. 492, to provide resold local exchange telecommunications services
within the State of Idaho. The Commission approved this name change to Certificate No. 492 by
Order No. 34784, in Case No. VGP-T-20-01.
