The Staff of the Idaho Public Utilities Commission, by and through its attorney of
record, Brad Purdy, Deputy Attorney General, requests that U S WEST Communications, Inc. provide the following documents and information, pursuant to Rule 225 of the Idaho Public Utilities Commission’s Rules of Procedure, IDAPA 31.01.01, on or before FEBRUARY 18, 1997.
This Production Request is to be considered as continuing, and U S WEST Communications, Inc. is requested to provide, by way of supplementary responses, additional documents that it or any person acting on its behalf may later obtain that will augment the documents produced.
For each item, please indicate the name of the person(s) preparing the answers, along with the job title of such person(s) and the witness who can sponsor the answer at hearing.
Request No. 1:Regarding your proposed revisions to the Access Service Tariff, please provide a summary recap of Rate of Applicable Rate Impact Statements showing all of your access customers and the amount of impact on each customer, listed by customer.  Please provide this list for northern Idaho and for southern Idaho.
Request No. 2:Please provide a detailed breakout of quantities and charges to access customers based on 1995 charges for your five largest access customers in each LATA.
Request No. 3:U S WEST has advised that the rates for transport and switching were cost based, while CCL rates were adjusted to maintain revenue neutrality.  Please confirm if this is correct.
Request No. 4: Please provide cost support date and the calculations you used to determine the cost basis for these items.
