USW-S-96-5 FOURTH PRODUCTION REQUEST OF THE COMMISSION STAFF TO U S WEST COMMUNICATIONS, INC. The Staff of the Idaho Public Utilities Commission (IPUC; PUC), by and through its attorney of record, Donald L. Howell, II, Deputy Attorney General, requests that U S WEST Communications, Inc. (U S WEST; USW; USWC; Company) provide the following documents and information on or before FRIDAY, FEBRUARY 7, 1997 at 4:30 P.M. This Production Request is to be considered as continuing, and U S WEST Communications, Inc. is requested to provide, by way of supplementary responses, additional documents that it or any person acting on its behalf may later obtain that will augment the documents produced. For each item, please indicate the name of the person(s) preparing the answers, along with the job title of such person(s) and the witness who can sponsor the answer at hearing. Provide responses in electronic form where possible, e.g., spreadsheets used to derive responses.  Any spreadsheets provided should include formulas rather than just values. All data should be for Southern Idaho only, and for the 1995 test year except where specified.  Basic local exchange service means Title 61 lines, residential or business. Request No. 269:  Please provide Staff with all correspondence, reports and workpapers provided to or received from the FCC and\or other state jurisdictions regarding Part 64 in 1994, 1995 and 1996. (MF) Request No. 270:  Please provide Staff with all workpapers, reports and analyses prepared that show FCC Part 64 items were removed from regulated rate base and expenses in 1994, 1995 and 1996.  (MF) Request No. 271:  Please provide the subaccount details for the summary accounts shown on STF000-006.  Identify the detailed accounts showing the amounts removed as non-regulated and/or Part 64 in 1995.  (MF) Request No. 272:  Please provide Staff with the amounts removed for the 1995 test year as non-regulated and/or Part 64 from the aircraft expense in Accounts 6113, 6711 and 6728.  (MF) Request No. 273: Please provide any and all studies that establish the Part 64 separations related to the CAAS allocation system as applied to 1995 separations.  (SL) Request No. 274:   Please provide any and all cost/analysis or studies related to intangible assets as the benefits related to southern Idaho Title 61 ratepayers.  (SL) Request No. 275: Please provide all rationale for why southern Idaho Title 61 ratepayers should pay for the U S WEST consolidations of Mountain Bell, Pacific Northwest Bell, and Northwestern Bell.  (SL) DATED  at Boise, Idaho, this            day of January 1997. ______________________________________ Donald L.
