REQUEST OF THE
COMMISSION STAFF
TO FARMERS MUTUAL
TELEPHONE COMPANY, INC.
The Staff of the Idaho Public Utilities Commission, by and through its attorney of record, Weldon B. Stutzman, Deputy Attorney General, requests that Farmers Mutual Telephone Company, Inc. provide the following documents and information on or before FRIDAY, SEPTEMBER 1, 1995.
This Production Request is to be considered as continuing, and Farmers Mutual Telephone Company, Inc. is requested to provide, by way of supplementary responses, additional documents that it or any person acting on its behalf may later obtain that will augment the documents produced.
For each item, please indicate the name of the person(s) preparing the answers, along with the job title of such person(s) and the witness who can sponsor the answer at hearing.
1.What rate will NuAcres multi-line customers be paying after the sale? Will they be paying the rates in effect before or after the multi-line reprice effective September 21, 1995?
2.How long does Farmers intend to charge the NuAcres customers the U S WEST rates?
3.Please provide audited financial statements for 1992 through 1994.
4.How many held orders were there in the area served by Farmers at the end of each month from January through June of 1995?
5.Please provide a set of the by-laws for Farmers Mutual.
6.Please provide the intrastate switched access minutes of use for Farmers. Please split these out by originating and terminating minutes as well as intraLATA and interLATA.
7.Does Farmers currently provide equal access?
8.If a customer who is currently a co-op member in Fruitland moves to NuAcres, what rate will the customer pay?
