All telephone corporations (except cooperative telephone corporations) formed since Janua/y 1 , 19B8 must file a notice
before offering telecommunjcation~ services in Idaho. These corporations include but are not limited to sellers of MTS or
WATS, resellers of MT$ or WA TS services. Operator Service Providers, Data Providers, etc. These notices must be
updated at least annually between Dacamber 1 and December 31 each year and whenever there ii;l a change in the
telephone corporations name, address or agent Plea$e e-mail the completed form to: secretary~puc.state.id.
