On March 17 2006 , KMC Telecom V, Inc. submitted a letter to the Commission
requesting its Certificate of Public Convenience and Necessity (CPCN) be relinquished and to
cancel its Price Lists in Idaho. The Company indicated it is no longer in operation and has no
customers in Idaho.
The Commission granted KMC Telecom V Certificate No. 423 in Case No.
KMC-03-01 on April 24, 2004, in accordance with Order No. 29426, issued on February 11
2004.
ST AFF RECOMMENDATION
Staff recommends the Commission grant the Company s request to relinquish its
certificate.
