Re: Midvale Telephone Company Local Access Tariff
IPUC Case No. MlDT-09-03: Order No. 30969
Dear Ms. Jewell:
As you know, I represent Midvale Telephone Exchange, Incorporated ("MTE") in the
above referenced matter. In connection with the above referenced order, and the transfer
of the Certificate of Public Convenience and Necessity from MTE to Midvale Telephone
Company ("MTC"), a wholly owned subsidiary, MTC was to resubmit tariffs in its name, as
opposed to MTE. MTE is one of the issuing carriers under the Idaho Rural Exchange
Carriers Access Service Tariff. With the permission of the Commission, we would like this
letter to serve as notice that wherever the name Midvale Telephone Exchange,
Incorporated is referenced in that tariff, the name should read Midvale Telephone Company.
At such time as any future changes are required in connection with such tariff, the
necessary changes will be made to the document in connection with the name change.
If this is not acceptable to the Commission, please let me know and we will revise
the tariff accordingly.
