Due to changes in the telephone industry, it has become necessary to initiate certain premises visit and time/
material charges when trouble is found to be in the customer s premises wiring or apparatus. In order to helpalleviate some of these charges, we have installed a device to help you isolate the trouble on your telephoneline before you call us,
This device is called a telephone NETWORK INTERFACE DEVICE (NID)which is located outside your house and is the point of separation
between your house and Frontier, .NOTE: All customers may not beequipped with an NID.
Should loU have trouble with your telephone service, the first thing wewould like you to do is make sure the trouble is not in your telephone
instrument (apparatus) itself.
STEP 1 Unplug your telephone inside your house,
STEP 2 Plug your telephone in at a friend's house. If it checks out OK it is time to check the inside wire,
(See STEP 5, Network Interface Device.
If a friend's house is not convenient and there is more than one telephone, unplug alltelephones and plug them back one at a time, listen for dial tone,
If no dial tone, unplug and plug a different set in a different outlet, If still no dial tone, check
your wiring as stated below, (See STEP 5, Network Interface Device.
At your Network Interface Device (located outside your house), take a standard blade
screwdriver and open the lid, In the upper left-hand corner you will see a modular plug, Byunplugging this cord, you are disconnecting the wire going inside your house,
Plug your known good telephone into this jack, If your telephone works properly here, but notin your house, the problem is in your inside wire,
NOTE: If you have two lines at your premise, a Network Interface Device with two testjacks wilf be provided, The first is wired to the red/green (1) terminals, the second lineIS wired to the yellow/black (2) terminals,

PROSECUTING FRAUDULENT CALLERS
It is illegal for another person to charge long distance calls to -
your numberor calling card without your permission, People
using unauthorized calling cards or phone numbers to avoid
charges are subject to prosecution and may be imprisoned
fined or both,
RESOLVING DISPUTES
If you have a dispute with your utility company that is not
resolved by contacting the company, the P,C.'s Consumer
Services Division is available to help you, You may

RESTORATION OF SERVICE
Unless prevented by circumstances beyond the company
control or unless the customer requests otherwise, Frontier
shall reconnect previously disconnected service by 5:00 PM
on the next business day following either:
. Receipt of the full amount in arrears for which service was
disconnected, or upon verification by Frontier that
conditions which warranted disconnection of service have
been eliminated,
. An agreement by Frontier and the customer on a Deferred
Payment Agreement and a payment, if required, under
the DPA.
SERVICE BYLAWS
. Equal Opportunity Provider: Frontier provides telephone
service without regard to race, nationality, color, religion
sex or marital status,
. Regulations: As a telephone company, Frontier is regu-
lated by the Idaho Public Utilities Commission, A complete
copy of rules and regulations is available at the State Office
and the Frontier Customer Service and Sales Center,
. Rates: Rates and practices included in this directory are
based on standards in effect at the time the directory was
published and are subject to change,
. Ownership of Telephone Numbers: All telephone num-
bers are owned by the telephone company. Frontier has the
right to change any customer's number as may be required,
. Employee Identification: For your protection, every
employee who visits a customer's home carries an official
employee identification card, It includes the employee
name and photograph, If you have a doubt about a person
who claims to be representing Frontier, ask to see the
person s identification card,

SUSPENSION OF SERVICE FOR
NONPAYMENT
The telephone company' will mail a written Notice of
Suspension to customers at least ten (1 0) days before the
date of the proposed suspension of service,
If your service is terminated, you may be subject to
additional charges involving reconnection fees and deposit
requirements in order to restore service,
Service will not be refused or suspended:
. For failure to pay for a different Class of Service from that
being provided to the location of the account.
. For delinquency in payment for service by a previous
occupant at the premises other than a current member
of the same household,
. For failure to pay an amount which is currently involved
in payment dispute and resolution procedures,
. For failure to pay directory advertising charges, toll
charges or any other nonregulated service charges,
USAGE BYLAWS
Harassment: Making threatening, obscene or nuisance
calls is a violation of state law, If you receive a threatening
phone call, report it immediately to the Police, If you
receive obscene or nuisance calls, follow these suggestions:
. Hang up
. Give no information
. Advise children to do the same
If calls continue, contact our Customer Service and Sales
Center for advice, Often, changing your number will
resolve the problem,
If the problem occurs continuously over an extended
period, contact the Frontier Annoyance Call' Bureau at
800-236-5688, for additional information, They can
be reached from 8:30 AM to 4:30 PM EST, Monday
thru Friday, If this is life threatening, call your local
law enforcement agency,
Emergency Interrupt: If a telephone number is constantly
busy, you can ask the operator to check the line, If the busy
signal is due to a conversation on the line or a receiver off
the hook, you will be charged an operator verificationcharge, In the case of an emergency, you can ask the
operator to interrupt the conversation,
Fraud: For your protection, Frontier is able to detect
fraudulent calls, State and Federal laws provide that no
person shall defraud or attempt to defraud the telephone
company (Frontier) of its lawful charges,
Use of Residence Phone for Business Purposes: Residence
phones are installed with the understanding that they will
be used for normal domestic purposes, A residence
telephone will be changed to business service if used for
business purposes or if it is advertised for business purposes,
Downgrades: If you are a business customer who wishes
to change from business. telephone service to residential
telephone service, you will be required to change your
telephone number, unless the downgrade is coincident
with the new directory publication,
Automatic Dialing-Announcing Devices: No automatic
dialing announcing device (ADAD) can be connected to
the Frontier network until certain requirements are met,

. A telephone solicitor is someone who calls
- you to ask you to buy a product or service
- overthetelephone.ldahostatelawrequires
.. that telephone solicitors, before making
. any other statement except a greeting;
state that the purpose of the call is to effect
.- a sale. Solicitors must identify themselves
state their tradename, and state the kind
of goods or service being offered,
Telephone solicitors are prohibited from
intimidating or tormenting any person of
n~rmal and reasonable se,:!sitivity, refusing
. hang up once asked to do so, or
misrepresent the price or quality of product
or service being offered.
Idaho state law requires that telephone
solicitors who meet requirements of
registration in the State to provide their
registration number to anyone who
requests it, orally inform the person at the
time the purchase is completed of his/her
right to' cancel, and give the full street
address, including the telephone number
of the telephone solicitor if a sale or
purchase is completed.
The Consumer Protection Unit of the
Attorney General's office enforces the Idaho
Telephone Solicitation Act. In addition
individuals who have suffered damages
may sue the telephone solicitorfor $1 ,000
or cost of recovery, whichever is greater

Statement of Billing Error Rights
This statement sets out your billing error rights with respect to telephone-billed purchases made using
interstate 900 pay-per-call services that are billed to you through Frontier,
The rights and obligations of you, the customer, and of Frontier and the long distance company who are
billing entities, set out here, are provided under the Federal Telephone Disclosure and Dispute Resolution Act.
This is the procedure that you must follow to notify Frontier of a billing error and the steps that we or the long
distance company must take in response to your notice:
You may provide notice of a billing error to Frontier by telephone or in writing. If you write, youshould:
1. give us your name or the customer s name, telephone number to which the charge was billed;
2, tell us the date and amount of the error, and the type of error you believe occurred;
3, tell us why you believe it was an error; and
. report the billing error no later than 60 days from the first billing statement that contains the
error you bE!lieve occurred,
You should contact Frontier first, since we have an agreement to receive billing error notifications from
customers, You may provide a notice to the long distance company instead, and it may ask you to contact directly, and provide to you our name, mailing address, and business telephone number, even if it also sendsyour billing error notice to us,
A customer who orally communicates an allegation of a billing error is presumed to have provided sufficient
notice to initiate a billing review,
Frontier will conduct a billing review, inform you and the billing carrier or vendor of the results of that review
and issue any appropriate credits, If we determine that no error occurred, we will upon request provide a
written explanation and proof of the indebtedness.
While we are conducting the billing review, you are not required to pay the disputed amount. However, failureto pay the amount determined not to have been the result of a billing error may be reported to a credit
reporting agency or subject you to a collection action. If you continue to dispute any portion of your billing
ern;,r claim, the fact your account is delinquent will be reported, If the dispute is subsequently resolved, thatfact will be reported to all who received the initial report that the account was delinquent,
If either we, as your billing entity, or the long distance company, as a billing entity, fail to follow the billing and
collection procedures prescribed by 308.7 of the Federal Trade Commission rule implementing the Federal
Telephone Disclosure and Dispute Resolution Act, we are obligated to forfeit any disputed amount, up to $50per transaction,
Disclosure Under FCC Rule 64.1509(B)
The FCC also requires disclosures to telephone subscribers, which are as follows:
Your local exchange and long distance service cannot be disconnected or interrupted as a result of your failure
to pay charges for interstate pay-per-call service, charges for interstate information services provided pursuant
to a presubscription or comparable arrangement, or charges you have disputed for interstate tariffed collectinformation services,
You can obtain blocking of access to services at no charge within 60 days after new service is established and it
becomes effective, Other requests for blocking, and requests for unblocking are subject to a reasonable one-time fee, Our tariffs include the terms and conditions that apply,
You have a right not to be billed for pay-per-call services not offered in compliance with Federal laws and
regulations established under Titles II or III of the Telephone Disclosure and Dispute Resolution Act,
Your access to 900 services may be involuntarily blocked for failure to pay legitimate pay-per-call charges,
