Original Page No. 26, 2.16.3.7: Please provide an explanation as to what the notifcation wil be to the customer for their failure to pay MTS charges. Even though the Company reserves the right to suspend service for continued non-payment, the ordinar process is to allow a Customer a number of billing cycles to make payment arangements. If undisputed amounts remain unpaid after several biling cycles, a Company representative contacts the Customer directly to discuss payment options. During these discussions, the Customer is advised that if payments are not made in accordance with any payment plan agreed to by the Company and the Customer, MTS service may be suspended or discontinued. Suspension or discontinuation of service is preceded by a written notification to the Customer indicating the date upon which (or after which) service will be suspended or terminated in the event payment is not received by the Company. The Company does not suspend or terminate service on any day preceding a weekend, a holiday or any day upon which the Company is not open to accept payments from Customers. Original Page No. 41, 3.3.5: In this section it states that there wil be "a charge wil apply when the operator verifes that the line is busy with a call in progress, the operator verifes that the line is available for incoming calls" - what is this charge and where is it located in the price list? The charges (residential and business) are located at section 4.7, page 70: Busy Line Verification, each request Busy Line Verification with Interrpt, each request Residential $1.49 $2.07 Business $2.50 $5.00 Original Page No. 58, EQUIPMENT RENTAL: The Commission does not regulate Customer Premise Equipment - please remove this section from the price list. Until the FCC determines whether this is intrastate services, we have declined to allow it into the intrastate price list for basic local exchange service. Please remove. Removed. Original Page No. 68 -72 Idaho Telecommunications Service Assistance Program (lTSAP): Please remove this section - the company is responsible for collecting this (as a line item charge) but it has been the experience of the Commission Staff that this charge changes annually and Companies often overlook the requisite price list changes. Therefore, it is easier to implement without the extra price list filings each year. Removed. Original Page No. 74,4.1.1 (Change of Telephone Number): Why is there an NRC assessed for a Telephone number? The nonrecurng charge assessed represents a combination of (i) Broadview's internal administrative costs associated with records and other system modifications necessitated by a change in telephone number requested by the Customer and (ii) costs imposed by Broadview's underlying carrer associated with assignent of a new telephone number. Original Page No. 76,4.1.2 (Change in Telephone Number): Again, please explain why there is a NRC assessed for the changing of a telephone number. Please see previous response. Original Page No. 88, 90. Why are there charges being assessed for blocks of telephone numbers? Where are these numbers coming from and what is an average customer going to be doing with blocks of 20? The Idaho Commission has jurisdiction over basic local exchange service for residential customers and business customers with five or fewer lines. Broadview provides service in many jursdictions to very large business customers which require significant numbers of individual telephone extensions. Broadview obtains these numbers from its underlying carers or requests Dils in large blocks in accordance with the North American Numbering Council's number resource utilization procedures. In light of the limitation of the Idaho Commission's exercise of authority over business customers with more than five lines, the 20-number block language has been deleted. Original Page No. 92 - 94, 4.13 - IP Service. This is a jurisdictional issue that is yet to be resolved at the FCC - please remove this section. The same question regarding blocks of 20 telephone numbers applies here as well. Section 9.13.6, "Broadspeed Dynamic IP", has been deleted. Please see previous answer with respect to 20-block numbers issue. Original Pages No. 95 - 98: Lifeline, Link Up America: As discussed above with ITSAP - these are applicable to the Companies' operations and for administrative ease; please remove. Removed.
