RE:QWEST'S TARIFF ADVICE NUMBER 05-01-N REQUESTING
COMMISSION AUTHORITY TO ADD UNIVERSAL SERVICE ORDER
CODE ("USOC") FOR THE FEDERAL ASSISTANCE PLAN CREDIT.
On February 3, 2005, Qwest filed its Tariff Advice Number 05-01-N to notify the
Commission that it intended to add a new USOC code for the Federal Assistance Plan.
Currently, Qwest's tariff uses a single USOC (ASGFX) to provide a credit of$IO.OO to
eligible subscribers of the FCC Lifeline Program. With the change, the Company will now
provide the credit based upon two USOC codes. They are as follows:

The amount of the credit will remain the same and this filing will be not affect customers.
Accordingly, Staff believes it is appropriate for the Commission to act upon the tariff advice
without notice of application being generally distributed to the public.
Although these USOCs are for federal credits, this filing also involves the State s portion
of the ITSAP credit. Staff recommends the Commission approve Qwest's Tariff Advice Number
05-01-
