RE:QWEST CORPORATION'S ADVICE NO. 04-04-S REMOVING BLOCK
COMPROMISE CHARGE
On August 25 2004, Qwest Corporation submitted Advice No. 04-04-S. In this filing,
Qwest seeks approval to remove the Block Compromise Charge associated with Direct Inward
Dialing (DID) Service in the Basic Local exchange Tariff for Southern Idaho. DID sequential
number block is a group of20 telephone numbers in numeric order. The last digit of the first
number within the block is a zero, and the last number within the number block must include an odd
number in the sixth digit and a nine in the last digit. A Block Compromise Charge of$50.00 for
permanent removal or $450.00 for temporary removal per sequential number block currently applies
when a customer requests a number be removed from the sequential numbering block. As in its
earlier Advice No. 04-04-, approved by the Commission on August 30, 2004, Qwest now
proposes to remove these charges from the Southern Idaho Tariff and has asked for an effective date
of September 1 , 2004.
