1. Introduction
Hydro One Networks Inc. (HONI) is committed to the principle that “All Occupational Injuries, Illnesses and
Fatalities are Preventable” and has embarked on a companywide campaign “Journey to Zero to reduce and/or
eliminate all health and safety incidents. HONI strongly believes that Contractor’s health and safety is an
essential part of its business and that incident prevention is a part of everyone’s job working on behalf of
HONI.
The Contractor is responsible for preparing a Project Specific Health and Safety Plan (PSHSP). This document is
a guideline for use by Contractors executing Hydro One Networks Inc. Engineering, Procurement,
Construction(EPC)/Procurement Construction (PC)/Construction(C) Agreement projects. The purpose of this
document is to define the structure and content for the PSHSP. The PSHSP is a safety and health policy and
program document to be used by the Contractor to facilitate the application of these processes and lead to a
more comprehensive understanding of requirements.
Once the contract is established, the life of the PSHSP will continue until the Contractor has successfully
completed all of its obligations under the contract. The Contractor is responsible for preparing the project
specific safety plan. A Contractor who has failed to submit an approved PSHSP will be issued a formal notice
to comply. Approval of the PSHSP is a prerequisite of the Contractor prior to the mobilization of the
Contractor and its Subcontractors to the Site.
2. Contractor’s Health and Safety Management Program
2.1 Application
The Contractor shall acknowledge HONI’s strong commitment to health and safety through its own written
Health and Safety Policy and Health and Safety Management Program which has been signed, dated and is
actively supported and endorsed by the Contractor’s management. The Contractor shall ensure that its
Health and Safety Policy and Health and Safety Management Program is widely disseminated and understood
amongst the Contractor’s and any Subcontractor’s employees for the Project. The Contractor shall ensure a
process of pre-qualification, selection and performance management is implemented and maintained for all
Subcontractors.
To ensure the Health and Safety requirements of the Contract are met, the Contractor must have, maintain
and comply with a Health and Safety Management Program which follows a recognized industry Health and
Safety standard such as OHSAS 18001:2007 or equivalent. The Health and Health and Safety Management
Program of the Contractor must be documented in a Health and Safety Program Manual. Contractors with
applicable certifications should include a copy of their Registration Certificate in their PSHSP.
The Contractor shall establish measures to regularly review audits, evaluations, regulatory requirements, HSE
performance, changing circumstances, and incident investigations, corrective and preventative actions to
ensure the continuing suitability, adequacy and effectiveness of HSE management on the Contract. The
Contractor shall have a copy of their corporate HSE program, the applicable HSE legislation, other applicable
regulations, and their Site specific HSE plans on the Site and available for review.

2.2 Health and Safety Policy and Program
The Contractor must provide both electronic and hard copies of its Health and Safety Policy and Program to
HONI in advance of Work. The Contractor is to ensure compliance of its personnel, at all times, strictly observe
and comply with all procedures. Any Health and Safety Rules or Procedures provided by HONI will supersede
the Contractors program. The Contractor must give prompt written notice to HONI of any objection of the
Contractor to the requested supplement, including the reasons for objection. HONI’s rights are not intended,
and must not be construed, to relieve the Contractor from any obligations to ensure compliance with all
provisions of the Contract.
2.3 Contractor’s Project Specific Health and Safety Plan (PSHSP)
The Contractor must prepare, implement and administer the Contractor’s PSHSP. The PSHSP is to be provided
in writing and accepted by HONI, prior to mobilization to the construction Site for Work under the Contract.
The PSHSP must comply with this document including any Project Site Rules, and applicable laws relating to
workplace health, safety and environmental health.
Any proposed amendments or revisions to the PSHSP are to be submitted to HONI for acceptance, and once
accepted, becomes part of the PSHSP.
3. Project Specific Health and Safety Plan Requirements
This guide document was developed to assist Contractors in the effective application of Health and Safety
Management Programs for EPC/PC/C Projects construction. It provides guidance and template material which
is intended to assist the Contractor’s relevant management or technical staff, in producing a PSHSP. It is also
useful background reading for anyone involved in developing or monitoring the PSHSP. The PSHSP shall
interface with HONI’s overall health and safety program. It contains an outline for a PSHSP attached as
Attachment A. In the PSHSP, any portions of HONI’s overall safety and health program that are referenced in
the PSHSP shall be included as an appendix as appropriate.
Unless otherwise set out in the Contract, the PSHSP shall be submitted to the HONI Contract Manager at least
two weeks in advance of the start of the project mobilization. The PSHSP must be submitted in electronic
form (as a PDF or Microsoft Word file) to the HONI Contract Manager and it will be reviewed by both the
HONI Health and Safety Manager and HONI Quality Assurance Manager. Any revisions to the plan will be
submitted back to the Contractor and the comments shall be incorporated into the PSHSP. A revised PSHSP
shall be returned to the HONI Contract Manager and kept in the Project file.
It is the responsibility of each Contractor working on HONI project Site to implement, enforce and modify
when necessary the health and safety policies and procedures identified in the PSHSP and herein.
Communication and training are an integral part of the program, and should be emphasized over the duration
of the project. In order to facilitate the above, every employee on Site shall follow the established policies and
procedures, report hazardous conditions, mitigate areas of concern before an illness and report injury near
miss or other incidents that are risk factors when realized. Contractors, as well as, other persons on HONI
project Sites are obligated to follow the same rules and regulations that have been implemented for
Contractors in accordance with the requirements of, but not limited to, the Occupational Health and Safety
Act.
In developing its PSHSP, the Contractor is encouraged to follow HONI’s format of this document and the
Outline attached as Attachment A. The PSHSP shall be written with the objective to provide project

management with easy access to health and safety requirements and should have ready availability of the
procedures and standards mentioned herein. The Contractor may elect to use all or part of the PSHSP Outline.
When using the PSHSP Outline, the Contractor shall make changes to section headings and text as needed to
meet project-specific requirements and the Contractor’s own Health and Safety approach. The PSHSP Outline
is provided for informational purposes only. HONI accepts no responsibility for the content of the PSHSP, nor
does HONI warrant that use of the PSHSP Outline will automatically result in Contract compliance. The
following describes the expected content for the sections in the PSHSP outline. The PSHSP must include, as
minimum the following information:
3.1 Signature Sheet
The Contractor shall provide a signature sheet containing title, signature and phone number of the following
personnel:
 Planner (qualified person such as corporate health and safety staff person)
 Plan must be approved, by company/corporate officers authorized to obligate the company (e.g., owner,
company president, regional vice president, etc.)
 Plan concurrence (HSM and Contract Manager)
3.2 Project Description
This part of the PSHSP must be specific to the project, highlighting main details such as project name,
purpose, duration, stages and health and safety considerations, on each phase of the project. This also
includes the definition of the Site location, layout plan and the stakeholders to be considered during the
course of the project.
In this section the Contractor shall provide a description of the key aspects of the project based on the main
stages identifying major activities and describe the nature and scope of work and relationship with the
ongoing processes at the Site location. This breakdown should be detailed enough to provide sufficient
information to perform the hazard and risk management assessment.
3.3 Statement of Health and Safety Management Policy
State the Contractor's Health and Safety Management Policy and how management assures that all EPC/PC/C
projects understand and implement Health and Safety Management activities. Identify all components of the
Health and Safety Team organization, (This may be done through an organizational chart). Discuss the
authorities of the Health and Safety Team personnel and demonstrate that the Team Members are qualified.
3.4 Responsibilities and Lines of Authorities
The Contractor shall define the Roles and Responsibilities of all personnel involved in the project which can
have an effect on the health and safety of the staff, workplace and the performance of the project, from the
tender process to completion of contract. This must include the name, position and appropriate authority of
personnel to ensure that health and safety requirements are met during the course of the project. It must be
clear who will implement and maintain Health and Safety practices, which are the health and safety
representatives on Site and who can make decisions and take actions on health and safety issues. Suggested
positions that should be defined with their Health and Safety responsibilities include:
 Corporate Management (President, Director, Project Director)

 Superintendent or Representative of the Contractor
 Contract Manager, Site Managers
 Health & Safety Management Representative

 Contractors and subcontractors
 Subcontractor’s information should include names, positions, contact details, health and safety roles and
responsibilities in connection with the project
3.5 Training, Orientation and Re-orientation
The Contractor shall describe the measures established to ensure that all Contractor Personnel on Site are
adequately qualified, suitably trained/certified, and sufficiently experienced to perform Work in a safe
manner with minimum supervision. The Contractor shall ensure all personnel must have successfully
completed the Stations Awareness Training acceptable to the HONI Contract Manager, before coming to Site.
The Contractor shall maintain training records for all employees. Those Workers who do not meet these
criteria shall be directly supervised by a competent Worker. Where required for the Contractor to safely
perform his or her Work, the Contractor shall ensure that the Worker is trained in the following, but not be
limited to:
 Station Awareness Training (mandatory)
 Confined space entry procedures when applicable
 Fall protection
 Scaffolding (understand the significance of tags)
 Use of aerial work platforms
 Applicable permits, confined space entry, control of hazardous energy, excavation, hot work
 Respiratory protection
 Chemical hazards
 Workplace Hazardous Materials Information Program (WHMIS)
 Waste management
 Spill control/cleanup
 Working alone procedure
 Basic firefighting (fire extinguisher usage)
 Powered mobile equipment operation (crane, forklift, scissor lift, etc.)
 Rigging and Equipment Shoring
 First aid/Cardiopulmonary Resuscitation (CPR) (in accordance to requirements of the applicable
legislation)

Orientation and Re-Orientation
All Contractor personnel on HONI project Site(s) must receive Contractor Site Specific orientation prior to
commencing Work (with respect to the project). Re-orientation shall be required if the Worker has been away
from the Site for a period longer than one (1) year. The Site specific orientation must include the following
(but not limited to):
 Work area(s) – including any areas of operational facilities which are “off limits”
 Emergency Assembly Areas
 First aid facility locations
 Lunch and wash facilities/trailers, times of breaks walk in and wash up time
 Specific Site Hazards
3.6 Health and Safety Communication
HSE Meetings (Toolbox Talks)
At a minimum, Contractors shall hold weekly HSE meetings. The Contractor shall ensure adequate
management and Worker representation in attendance, including Subcontractors. The meetings must include
a HSE topic, Site specific health and safety concerns and identified corrective action, responsibility and
timeline. Minutes of these meetings must document attendance, identified areas of concern and determined
resolution methods. The minutes of these meetings shall be forwarded to the HONI Contact Manager.
Tailboard Meetings
Contractors must ensure that a daily Safe Work Plan takes place at the start of each shift or task. The meeting
must address the specifics of the task, a hazard assessment, controls required, permits required, and other
pertinent health and safety/task related information. If the task requires revision to the Work methodology,
then a new Tailboard Meeting must be conducted to address the changes and their potential HSE concerns.
Safe Work Plan shall be documented and documentation shall include, Worker attendance, concerns raised
and hazard controls required. Any changes are to be made in red ink and all workers to initial accepted.
Contractor HSE Management Report
The Contractor shall maintain project related HSE Incident records when working on HONI Sites. The
Contractor shall record all injuries, occupational illness, environmental spills or releases, near-miss Incidents
and hours worked.
Where required by HONI, the Contractor will provide HONI a monthly report on the above information to be
used to develop HSE related statistics. The following statistics will be determined in accordance with
established formulas (Canadian Electrical Association or OSHA Bureau of Labour Statistics) for:
 Accident
 Incident
 Lost Time Injury Frequency (LTIF)
 Lost Time Injury Severity (LTIS)
 All Injury Frequency (AIF)

 Medical Treatment Injury (MTI)

 Restricted/Modified Duty Severity (RWS)
 Near Miss Notifications
Where a program of leading indicators has been established for a project, the Contractor shall collect and
maintain records on the leading indicator performance. The development of leading indicators shall be
determined by HONI and the Contractor based on the Hazard identification/risk analysis performed for the
project.
3.7 Operational Control
The Contractor shall ensure operational controls are in place for the scope of the Work being performed.
Examples include procedures for demolition, hot work, and work over water, ground thawing, pesticide
application, radioactive material, lasers, electrical or substance isolation (blanking, lockouts), carcinogenic
material and other Hazards as specified by HONI. These procedures shall be made available to the HONI
Contract Manager. The Contractor shall ensure that these operational controls outline safe work practices
that address HSE concerns. Workers on the Site that are exposed or potentially exposed to these Hazards shall
be familiar with and follow the prescribed controls. This includes but is not limited to the following categories:
Codes of Practice
The Contractor shall train and familiarize all personnel involved in the project under their control of the
contents of the codes of practice. If there are no codes of practice in place (new construction Site) then the
Contractor shall develop appropriate codes of practice in accordance to legislated provincial requirements
and OHSA guidelines. The Contractor shall post at all Sites codes of practice if the Work includes:
 Confined space entry
 Respiratory protective equipment
 Designated chemicals (e.g., asbestos, lead, silica)
 Others as required under the applicable OHSA legislation
Each code of practice must reflect the specific operation conducted at that particular Site.
Document Control of Health and Safety Related Activities
The Contractor shall ensure all HSE management activities are documented and maintained for one (1) year
after project completion. Recordable incidents to be kept for seven (7) years. Such documentation must be
completed on the Site for audits. Examples of required documents are:
 Incident investigations
 Audits and planned inspections
 Health and Safety meetings
 Standards

Equipment Use, Maintenance and Repair
The Contractor shall ensure that all equipment used on the Site will not compromise the health and safety of
Workers, is maintained in good operating condition, and is only used in accordance with the manufacturer’s
recommendations by Competent Workers trained and qualified to use such equipment. Equipment shall not
be modified without the manufacturer’s written approval and all equipment shall be inspected prior to use.
Hand and Power Tools
The Contractor shall ensure that power and hand tools are properly used and maintained in accordance with
the manufacturer’s instructions and that all applicable guards and handles are in place. Tools shall be
operated only by Competent Workers who are knowledgeable in the safe use, limitations and maintenance of
that tool.
Health
The Contractor shall establish appropriate health procedures to address issues such as:
 Ergonomics/musculoskeletal injury
 Employee fitness to work
 Exposure to occupational Hazards such as noise, chemicals, radiation, etc.
Housekeeping
The Contractor shall ensure that the Site is kept clean and free from accumulation of debris and other Hazards
that may endanger Workers or restrict safe access or egress, including the proper storage of equipment and
material.
Hygiene
The Contractor shall ensure that exposure to chemical, biological, radiation and noise occupational Hazards
which are or may be present at the Site are adequately controlled to protect the health and health and safety
of Workers. The Contractor shall develop control plans which include Hazard assessment, Worker education,
safe Work practices, clean up, decontamination and disposal as required.
Ladders and Scaffolds
The Contractor shall ensure that all scaffold erection is in compliance with the applicable legislation in the
jurisdiction in which the Work is being performed and according to manufacturer’s instructions. All scaffolding
shall be properly tagged for use in accordance with legislative requirements.
All portable ladders shall be in good repair, and used in accordance with the OHSA requirements of the
jurisdiction in which the Work is being performed and Canadian Standards Association (CSA) Standards.

Ladders must be tied off at the top and secured at the bottom unless held in position by a second Worker.
Workers shall not perform Work from either of the top two rungs or steps of a ladder. Extension ladders shall
extend a minimum of three rungs (or one metre) beyond the platform, deck, roof line, etc.
Lunch Room and Sanitary Facilities
Unless the Contractor has specific permission to utilize the Site facilities provided by HONI, the Contractor
shall provide appropriate lunchrooms and sanitary facilities in accordance with applicable legislation. The
location of such accommodations shall be approved by the HONI Contract Manager before they are placed.
The accommodations shall be regularly cleaned and shall remain free of any form of contamination.
Movement of Equipment and Materials
The Contractor shall ensure that loads and materials are secured in accordance with load securement
requirements to prevent unintentional movement or spillage. The Contractor shall ensure Workers are aware
of the Hazards associated with working around moving equipment and that appropriate measures are taken
to protect the Workers from injury. Workers working in the vicinity of mobile equipment shall wear Canadian
Standards Association (CSA) approved Class II high visibility clothing or vests.
Movement of Manpower on Site
The Contractor will provide designated walkways to minimize exposure to vehicular traffic and excavations.
Non Conformance, Corrective and Preventative Action
While working on HONI Sites, the Contractor shall have an ongoing process to identify actual or potential
items of non-conformity and take corrective/preventative action to address the root cause of such items.
Where a risk assessment has determined new or changed Hazard or control requirements, preventative or
corrective actions shall be developed, communicated and implemented. The HONI Contract Manager shall be
provided with a record of such actions.
Office Health and Safety
Where the Contractor establishes administrative offices on the Site to support the project Work, the
Contractor shall establish a health and safety program specifically designed to mitigate hazards and risks in
the office environment.
Permits
The Contractor shall ensure that all permits are in place as specified in the Contract. Examples of permits are:
 Control of hazardous energy (e.g., lockout/tag out, grounding)
 Confined Space Entry
 Hot work
 Ground disturbance (excavation/trenching)
 Embedded Services Permit
The Contractor shall ensure that their permitting process complies with all legislated requirements and meets
the standards established by HONI. Personnel involved in the permitting program shall be trained in the
permitting process and shall follow the permit requirements for isolation, application of locks, testing of
isolation, air monitoring, rescue requirements, fire prevention requirements, cut backs, etc.

Personal Protective Equipment (PPE)
The Contractor shall ensure that their Workers and all Subcontracted Workers use appropriate PPE, high
visibility Flame Resistant clothing and are trained in its use, care, and limitations in order to control or
minimize Hazards that cannot be controlled by elimination, engineering or administrative practices.

Powered Mobile Equipment
The Contractor shall establish measures to ensure that powered mobile equipment meets the requirements
of applicable legislation in the jurisdiction in which the Work is being performed. Personnel must not be
transported in a vehicle unless adequate seating is provided. Equipment fitted with rollover protective
structures shall be equipped with seat belts and the seat belts shall be worn when the equipment is operated.
Equipment requiring back-up alarms shall have the alarms maintained in good working order.
Public Safety
The Contractor shall ensure that access or movement at or adjacent to the Site is controlled on a 24-hour
basis to ensure that the public or other Workers are not exposed to Hazards present at the Site at any time,
including during lunch and coffee breaks. This may involve the use of fencing, barricading, barrier tape,
lighting, signage, hoarding, and locked covers over openings, security personnel, “authorized entry only”
provision, or other means as appropriate. The Contractor shall ensure that authorized HONI personnel have
access to the Site.
Temporary Heating and Ventilation
The Contractor shall ensure that the install equipment and sufficient ventilation are provided at all times. The
fuel supply and heaters shall be secured to prevent falling over and protected from being struck. Temporary
ventilation that is provided to control levels of airborne contaminants or to ensure adequate levels of
respirable air shall be designed and maintained as prescribed in the Ventilation Regulation in force in the
Province of Ontario (or the analogous regulatory scheme in force in the jurisdiction in which the Work is being
performed).

Traffic Hazards
The Contractor shall establish processes and procedures to ensure that appropriate measures are taken to
protect Workers from the Hazards created by traffic or mobile equipment, including appropriate barricades
and the wearing of high visibility clothing or vests (CSA Class II or as required by the highway traffic standard
in the jurisdiction in which the Work is being performed) where required.
Signs, Warnings, Barricades & Barrier Tape
The Contractor shall erect “Danger” signs where an immediate Hazard exists and shall erect “Caution” signs to
warn against potential Hazards or to unsafe conditions. Barricades shall be erected to warn Workers and
public where danger exists.
Site Access/Security
The Contractor shall ensure that access or movement at or adjacent to the Site is controlled on a 24-hour
basis.

Welding and Cutting
All welding and cutting shall be done by Competent Workers in accordance with legislated requirements in
the jurisdiction in which the Work is being performed. Workers shall follow Work practices that prevent fires
and injury to themselves and other Workers at the Site such as, fire blankets and barriers shall be used to
protect other Workers and control fire hazards. Hot work permits and quality assurance procedures may be
required at some Sites.
Violence in the Workplace
The Contractor shall ensure that a policy/procedure is in place to address violence at the Site and in the work
place to comply with applicable legislation. The policy shall cover, but not be limited to:
 Harassment
 Threats and threatening behaviour
 Physical or mental abuse
 Investigation of suspected violence in the work place issues
 Disciplinary policy for violence in the work place
3.8 Hazard and Risk Management

The Contractor shall establish measures that describe the process to identify foreseeable health and safety
hazards assess the risks and define the control measures to be implemented during the course of the project.
Procedures are to be implemented to ensure that the hazards are controlled in the most effective and
efficient manner. The Contractor has an obligation to eliminate the hazards. If elimination of the hazard is not
“reasonably practicable” the Contractor must control the risk by implementing measures to lessen the risk of
harm to the lowest possible level.
The hierarchy of control is recommended to be considered until a control measure or a combination of
measures can achieve the required reduction in exposure. Hierarchy of control measures are as follows:
 Elimination
 Substitution
 Isolation
 Engineering Controls
 Administrative Controls
 Personal Protective Equipment (PPE)
The PSHSP must include a risk management process which includes hazard identification, risk assessment and
mitigation procedures to be applied to the project. A project risk register should be developed identifying the
above requirements and using the hierarchy of control to minimize the risks. All high risk construction work
activities on the project must be listed in the risk register.

3.9 High Risk Hazards
Asbestos
The Contractor shall establish measures to ensure asbestos containing materials (ACM) such as duct lines,
water mains, cable insulation, packing, or gaskets are to be used, appropriate procedures must be utilized to
handle the material, including cutting, shaping, installing and/or removing the ACM. The procedures
developed and utilized shall comply with the appropriate legislated requirements for the jurisdiction where
Work is being performed.
Asbestos Removal
The Contractor shall ensure that when Work requires the removal of asbestos material, such removal is to be
executed by trained abatement personnel only. Job specific procedures shall be developed in accordance with
applicable OHSA requirements for asbestos abatement, for the jurisdiction in which the work is being
performed.
The asbestos abatement Work must be supervised by a competent occupational hygiene consultant. Air
sampling must be conducted as per provincial OHSA legislative requirements, and HONI must be provided
with copies of all related documentation including notice of project, air sampling results, and waste manifests.

Chemical Hazards
The Contractor shall ensure that all controlled products which are brought onto the Site as part of the project
are stored, used and handled in accordance with manufacturer’s recommendations and WHMIS spell out first
time requirements. Chemical substances regulated by the Transportation of Dangerous Goods Regulation
(TDG) or the analogous regulatory scheme in force in the jurisdiction in which the Work is being performed,
shall be transported in accordance with that regulation.
Contractors shall utilize secondary containment around chemical or fuel storage containers. Absorbent
material capable of containing and managing small spills shall be available and accessible at all times. Spills of
chemicals in quantities deemed reportable under applicable legislation shall be immediately notified to the
HONI Health and Safety Manager and Site inspector with a follow-up Incident report.
Compressed Gases
The Contractor shall ensure that:
 Compressed gas cylinders are stored and used in accordance with regulatory requirements
 Compressed gas cylinders shall not be transported in elevators with any other personnel present other
than the person moving the cylinders with the exception of Self Contained Breathing Apparatus (SCBA)
and small containers of aerosols for non-destructive testing (NDT)
 Compressed gas cylinders shall not be taken into confined spaces
Lead Paint
Lead paint was used in older HONI facilities and may be found on equipment which was
coated for corrosion control. If a concern arises about the content of the paint or coating, a test
to determine lead content must be performed. Where paint containing lead is being removed
from existing equipment, piping, etc., the Contractor shall ensure that any dust is properly
controlled.

Material Safety Data Sheets
The Contractor shall have all Material Safety Data Sheets (MSDS) accessible to all Workers at the Site for
controlled products brought onto the Site for the project. The Contractor shall provide copies of the MSDS to
the HONI Contract Manager.
Use, Monitoring and Storage of Flammable Substances
The Contractor shall ensure that the Hazards associated with the use of any flammable substance are clearly
evaluated and procedures put in place to ensure the health and safety of Workers, materials and equipment
on Site. Flammable substances shall be stored in approved containers and rags contaminated with flammable
substances shall be stored in suitable metal containers with adequate covers. Sparks from “hot work”
operation shall be controlled in a manner which prevents fire and Worker injury.
Contractors shall ensure monitoring methods for applicable airborne contaminants, such as
hydrocarbon or chemical vapors and dusts are conducted if necessary and shall take immediate
corrective action if monitoring shows unacceptable levels of airborne contaminants. Should
monitoring of these contaminants be necessary, only Workers who have demonstrated
competence in monitoring shall carry out such Work and keep all appropriate records for review
by HONI.

Contactor shall develop gas monitoring equipment procedures for hygiene and/or confined
space entry that include maintenance, calibration and use (including pre-use bump testing) in
accordance with the manufacturer’s instructions. Instruments which require service or are
defective shall be immediately taken out of service, tagged, and removed from Site for
repair/calibration before reuse.
Workplace Hazardous Materials Information System (WHMIS)
The Contractor shall ensure that all controlled products present at the Site are identified with either WHMIS
approved supplier or Site labels.
3.10 Construction Hazards
The Contractor shall establish measures to ensure that all underground utilities are located in accordance with
the applicable legislation in the jurisdiction in which the Work is being performed. Specifically, the procedures
mandated by Ontario Underground Infrastructure Notification System Act 2012 and outlined in the Pipeline
Act for Ontario Electric Utilities Act for Ontario Electrical Utility Code (or the similar legislative schemes in
force if the Work is being performed outside of the Province of Ontario), and other any applicable legislation
in the jurisdiction in which the Work is being performed must be followed.
The Contractor shall ensure that Workers are made aware of the location of overhead and/or underground
utilities and that the dangers of contacting these utilities are communicated to all Workers. The Contractor
shall ensure that the safe limits of approach as outlined in legislation for Workers and equipment are
maintained at all times.
When it is necessary for equipment to move underneath overhead power lines the Contractor shall ensure
that the equipment is moved in a configuration which would prevent contact with the overhead lines. Signs
warning of the danger of overhead power lines shall be erected below the lines when travel under the lines is
required.

Excavations and Trenches
The Contractor shall ensure that excavations are conducted in accordance with the applicable legislation in
the jurisdiction in which the Work is being performed and take into account the locating and exposing of
underground utilities, soil conditions, Worker access/egress, spoil pile location, and barricading the
excavation. The Contractor shall ensure the protection of the Workers from cave-ins by cutting back the walls
and/or by installing temporary protective structures in trenches and excavations in accordance with
applicable legislation.
Fall Protection
The Contractor shall ensure that a fall protection plan is developed for a Site where a Worker may fall three
metres or more and is not protected from falling by guardrails. Workers shall ensure 100% tie off including the
use of double lanyards when required. The fall protection program shall be used in accordance with the
manufacturer’s instructions and shall be attached to an anchor point which complies with the legislative
requirements. Workers required to wear fall protection shall be competent in its inspection and use.
Openings that create a falling Hazard must have a temporary cover with warning signs or guardrails and toe
boards installed by the Contractor before the opening is created. Similarly, where open trench Work is
conducted guardrails or barricades shall be erected and maintained as required.
Hoisting and Rigging
The Contractor shall ensure that all aspects of hoisting and rigging comply with the applicable legislation in
the jurisdiction in which the Work is being performed. Only Competent Workers, designated by the
Contractor, shall operate hoists, act as signalers, or perform rigging. The operator of the lift device shall have
access to sufficient load charts and be made aware of the load weight to perform the lift safely. Log books for
cranes and hoists shall be maintained as required. Approved rigging shall be of sufficient strength, inspected
thoroughly at the beginning of each shift and used in a safe manner. Tag lines shall be used where required to
control the load to prevent undesired movement, unless the use of tag lines create a greater Hazard. At no
time shall loads be suspended over Workers, or travel over Workers. All rigging, manual lifting devices (chain
falls, etc.) and powered lifting devices shall be maintained, inspected, and used in accordance with
manufacturer’s instructions and the legislative requirements of the jurisdiction in which the Work is being
performed.
Manhole and Vault Covers
The Contractor shall ensure that when Work requires the temporary removal of a manhole or vault lid, that a
suitable guard is installed before the manhole lid is removed. The manhole or vault shall not be left
unattended while open. The manhole or vault shall be adequately protected and covered if it becomes
necessary to leave it unattended without its lid in place.
3.11 Emergency Preparedness and Response
Emergency Response Planning
Emergency Response Plans (ERP) are specific to each project, based on the Hazard identification performed by
the Contractor, an overall ERP shall be established (routes, equipment, emergency contacts, etc.) if required
to manage both man made and naturally occurring Hazards. Contractors shall inform all the Workers on the
Site of the contents of the plan. If the Site has an existing ERP, the Contractor shall coordinate the ERP with
the existing facility plan and shall familiarize all the Workers under their control of the contents of the plan.

The plan should be posted on Site in the Contractor’s trailer or any other location that is accessible and visible
to all employees.
Fire Protection
The Contractor shall ensure that an adequate number of the appropriate types and size of fire extinguishers
are provided and maintained in accordance with applicable legislation in the jurisdiction in which the Work is
being performed. The fire extinguishers shall be clearly visible and readily accessible. Workers shall be trained
in the use of the fire extinguishers provided. The extinguishers shall be inspected monthly and replaced if
missing, defective, or used.
First Aid
Contractors shall provide first aid services, equipment, and supplies in accordance with the current OHSA
legislation, taking into consideration the Work being performed, the number of Workers present and
response time for emergency response personnel. Throughout the duration of the Contract, the Contractor
shall ensure that the required number of qualified emergency first aid kits and standard first aid personnel are
present at the Site while work is being conducted.
Severe Weather
The Contractor shall have a procedure in place to effectively manage Workers who are exposed to extreme
temperatures. The procedure shall detail when the procedure is to be put into effect and shall outline specific
actions to be taken, including warm up or cool down periods, and when Work will cease entirely. If applicable,
the Contractor shall develop severe weather guidelines that include procedures for ground work only or Work
shut down rules. The severe weather guideline must include, but not be limited to:
 Extreme cold
 Extreme heat
 Severe rain, snow, hail
 Lightning
 Severe wind/tornadoes
3.12 Incident Investigation
The Contractor shall maintain an Incident management program that complies with applicable legislation in
the jurisdiction in which the Work is being performed. Contractors must provide Incident scene management
for situations occurring within their Contractual requirements. The Contractor must ensure that all Sites
supervisory staff has been provided with the training required for implementing Incident scene management
and reporting.
Occupational Health and Safety and Environmental Regulatory notification will be completed in accordance
with applicable federal, provincial or municipal reporting guidelines.

3.13 Records Management
Measures shall be established to maintain related HSE records and these records shall be made available to
the HONI Contract Manager upon request. HSE records shall include, but not be limited to:
 HSE related reports
 Certificates of competency
 Certificates or documentation of calibration and or bump testing for instruments
 Training records
 Inspection reports (workplace, environmental, etc.)
 Inspections and/or orders from regulatory bodies
 Audit reports and corrective action documents
 Other documents generated by the management of HSE directly related to the project
 Documentation of any meeting(s) held with Subcontractors to communicate HSE
 Request for Inspection responses and validation of completion HSE meeting minutes, Tailboards, Hazard
Assessments
Records Creation and Retention
The Contractor shall ensure that all appropriate HSE related records are created as required and a retention
procedure is in place. (See Section 4.7 Operational Control - “Document Control of Health and Safety Related
Activities”) for specific retention periods.
3.14 Audits and Inspections
Inspections
Where required, the Contractor shall establish a program of HSE related inspections to be completed weekly.
All findings shall be documented and a report detailing the proposed corrective action to be taken, including
time lines and responsibilities for completion.
HONI’s Contract Manager must be immediately notified of any inspections or orders by regulatory bodies
along with an action plan to address any identified deficiencies.

Contractors must conduct Work observations and/or inspections to review the health and safety and
environmental aspects of the task being observed. The purpose of these observations is to ensure that
Workers understand and apply safe work processes and procedures, as well as to establish a two-way
dialogue between supervision and Workers to discuss health and safety related issues.
Project HSE Auditing and Assessment
Contractors shall provide a program to accommodate periodic audits, assessments or inspections that
includes documentation, records review and interview plans with dates and assigned responsibilities.
Contractors shall perform internal HSE audits and inspections as a part of leading indicator activities to
measure health and safety related performance; Subcontractors shall be included in such audits. Audit
findings shall be documented and plans shall be developed to address the findings with immediate and long
term corrective actions identified, responsibilities assigned and completion dates determined. Copies of
audits and inspections shall be copied to the HONI Contract Manager and/or HONI Site Inspector for review.

4. HONI Role and Authority
The HONI management team is responsible for establishing and demonstrating commitment to the HONI
Health and Safety Policy (HHSP). The HONI management team is charged with ensuring that the HONI Health
and Safety Policy is understood, implemented and maintained throughout all appropriate levels of design and
construction and implementation, including any HONI group that participates in project activities.
The Contractor PSHSP provides specific requirements for program implementation, including the assignment
of primary responsibility for implementation.
It is the policy of HONI management team that projects that are executed by Contractors shall be planned,
designed and constructed with the highest regard for health and safety. The PSHSP submitted to HONI for
projects by contractors, subcontractors, consultants and sub-consultants, shall meet the mandates outlined in
the Contract documents and be executed independently of cost and schedule project functions.
4.1 HONI Health and Safety Manager (HSM)
The HONI health and safety manager has overall responsibility for the Contractor’s adherence to the Project
Specific Health & Safety Plan.
The Contractor will provide the PSHSP for review and the HSM will provide comment on completed
Contractors Project Specific Health and Safety Plans. During Contract execution, the HSM will assist, review
and provide advice as required.
4.2 HONI Contract Manager/Contract Administrator (CM)
The HONI Contract Manager (also known as the Project Manager and/or the Owner’s Representative), will
review the PSHSP for their respective projects, correspond regularly with the appointed HONI Site Inspector
and inform any incidents on Site to the HSM.

The HONI CM will conduct formal and informal site inspections to ensure program requirements are being
met by the Contractor and Subcontractors.
4.3 HONI Site Inspectors (SI)
The HONI Site Inspector shall be responsible for reviewing compliance on behalf of HONI of the Contractor’s
Project Specific Health and Safety Plan.
The HONI SI will monitor all Contractor field activities to ensure compliance with HONI and project specific
policies, standards, and requirements on behalf of HONI. If necessary the Site Inspector will inform the HSM
and HONI Contract Manager of the need for an additional investigation into safety related activities.

Definitions

Accident
An incident which has given rise to injury, ill health or fatality.

Competent Worker
In relation to specific work, means a worker who (a) is qualified because of knowledge, training and
experience to perform the work, (b) is familiar with the Occupational Health and Safety Act (OHSA) and
with the provisions of the regulations that apply to the work; and (c) has knowledge of all potential or
actual danger to health and safety in the work.

Constructor
A person who undertakes a construction project for an owner and includes an owner who undertakes all
or part of a project by himself or by more than one employer.

Contract(s)
All oral or written agreements, letters, memoranda, purchase orders or other documentation evidencing
a legally binding agreement between HONI and one or more third parties under which legally binding
rights, duties, obligations, commitments or promises are exchanged for consideration.

Contractor
A business or individual that is retained to perform Work, supply equipment, materials or general
construction or maintenance services and is not deemed an employee of HONI.

Contractor Supervisor
A person designated by the Contractor as their representative to direct the Work and ensure HSE
requirements are met by the Contractor’s Workers and/or Subcontractors.

Employer
A person who employs one or more Workers or who contracts for the services of one or more Workers
and includes a contractor or subcontractor who performs Work or supplies services and a contractor or
subcontractor who undertakes with the owner, constructor, contractor or subcontractor to perform
Work or supply services.

Environmental Aspect
An element of an organization's activities, products or services that can interact with the environment.

First Aid Injury (FAI)
An injury that requires first aid. Self-treatment of minor cuts/scratches by the employee is not
considered first aid accident (i.e. first aid is treatment provided by a first aider).

Hazard
A source, situation or act with a potential for harm in terms of human injury or ill health, or a
combination of these.

Health and Safety Policy
Overall intentions and direction of an organization related to its HSE performance as formally expressed
by top management.

HSE Meeting
Formal meeting held by supervisors with their Workers (including Subcontractors) to communicate HSE
requirements and address concerns.

Incident
An undesired event that has, or could have, resulted in personal harm, property damage, environmental
damage or loss.

Leading Indicator
Leading indicators measure the direct and indirect precursors to harm, and give advance warning before
an event occurs that might lead to an undesired outcome, providing an opportunity for preventative
action to be taken.

Lost Time Injury (LTI)
For the purposes of WSIB reporting, an injury where an employee loses work time beyond the date of
injury as a direct result of an occupational illness/injury.

Medical Treatment Injury (MTI)
An injury requiring treatment by a medical practitioner that is beyond normal first aid and that is not the
result of a pre-existing condition.

Near Miss
An event which, except for chance, has the potential to result in an injury or equipment damage.
“Potential” means there must be (i) an unwanted energy flow sufficient to cause harm and (ii) the
control of this energy flow is less than adequate (eg. ineffective, weak or missing barriers).

Owner’s Representative
HONI employee who is primarily responsible for ensuring the completion of a defined scope of work by a
Contractor.

Prime Contractor
Contractor, employer or other person who enters into an agreement with the owner of the Site to be the
Prime Contractor; or if no agreement is in force, the owner of the Site is the Prime Contractor.

Project Specific Health and Safety Plan
A written plan that shall describe compliance strategies for all applicable health and safety legislation,
foreseeable job site hazards and precautionary measures, prerequisite employee health and safety
training, safe work procedures, standard operating procedures (SOP’s), first aid preparedness, stop work
procedure, emergency response plans, incident reporting procedures, and the names and contact
numbers of job – health and safety supervisor(s).

Services
Means all labor, duties, functions, and activities required to be performed by the Contractor under the

Subcontractor
A contractor who does not have a direct Contract with HONI but is under the direction and responsibility
of a Contractor who does have a direct Contract with HONI.

Tailboard Meeting
A meeting conducted at the HONI Site by the Workers to assess the adequacy of the job plan and to
ensure that all associated hazards/risks have been identified and controls established.
