I ("Customer"), hereby authorize Idaho Power Company to release to the Weatherization Inspector the following information regarding
my Customer Service Account: phone number and address of home where efficiency improvements have been installed and
weatherization measures installed in my home to be inspected. Customer understands that the sole purpose of this information is to
allow the Inspector to verify efficiency improvements have been installed to company standards. Customer hereby releases and
discharges Idaho Power Company, its officers and employees, from all claims and liabilities, which Idaho Power Company might
otherwise incur as a result of the release of information hereunder. I also certify that I am the individual whose name appears on the
account.
