Your November Energy Serices Agreement invoice D1111002 in the amount of$1,895,656.26
was due Wedesday, December 21st. As ofthe morning of December 2200, payment' has not
been received. Per IPUC's Utilty Cutomer Relations Rule 602, failure to pay an undisputed
bil is grounds for termation of serice. As a result of your past due payment, Hoku's energy
serce wil be terated on the morng of Thursday, December 29th uness payment is
received prior to 5 p.m Mountai Time on Wednesday, December 28th.
An informal or forml complaint concerng thi teration may be :fled with the Commsion.
Service wil not be terated prior to the resolution of a fied complait.
