Employment Impacts
1-1 Provide any company guidelines, policies, and requirements that apply to the
use of contractors—who perform traffic control, paving, patch paving, utility
excavation, and the installation, maintenance, or rehabilitation of underground
utility infrastructure—in effect at any time since 2014.
1-2 Provide any company guidelines, policies, and requirements regarding labor
policies that cover the contracted out workforce, who work for contractors—
who perform traffic control, paving, patch paving, utility excavation, and the
installation, maintenance, or rehabilitation of underground utility infrastructure
—at any time since 2014.

1-4 For historic capital investments, detail proportion of actual spending and
proportion of projects performed by outside construction contractors annually
from 2014 to present. Include original data and methodology to compute the
amounts and percentages.

out projects and the scopes of work to be performed by outside construction
contractors from 2017 to 2021.
1-7 Detail any changes to processes or systems post-merger as it relates to the

1-8 Detail any employment goals or commitments as it relates to contractor
employees in effect at any time since 2014.
1-9 Provide any analysis, document, or report since 2014 that details contractor
employment creation or outcomes as it relates to historic and future capital

1-10 Detail any contractor or procurement goals as it relates to Woman Business
Enterprises and Minority Business Enterprises.
1-11 Provide any document, analysis, data, reports, or surveys (completed at any
time since 2014) used to determine occupational wage data for contractor
employees and projected contract labor expenditures.
1-12 Provide copies of the last five most recent contract awards for the provision of
traffic control services. Please include the Request for Proposal, technical
evaluations used to qualify contractors, bid amounts and actual costs of the
awarded contract, and any and all performance evaluations.
1-13 Please provide copies of any and all portions or excerpts of current Master
Service Agreements, relating to safety requirements, compensation
requirements for workers, or other labor requirements, between the applicants
and outside contractors. Please provide a schedule of when active MSAs expire,
and the timeline for securing new MSA contracts.
Safety and Reliability
1-14 Provide copies of third party damage reports caused by, attributed to, or
investigated as being caused by or attributed to, contractors—who perform
traffic control, paving, patch paving, utility excavation, and the installation,
maintenance, or rehabilitation of underground utility infrastructure—from 2014
to current, and any reports or root cause analyses performed on third party
damages caused by, attributed to, or investigated as being caused by or
attributed to, contractors, over the same period.

1-16 Copies of any and all exhibits, data responses, testimonies, work-papers,
communications, or other documents that detail the prequalification process by
which outside construction contractors—who perform traffic control, paving,
patch paving, utility excavation, and the installation, maintenance, or

capital or maintenance projects.
1-17 Copies of any documents used to assess the qualification of construction
contractors—who perform traffic control, paving, patch paving, utility
excavation, and the installation, maintenance, or rehabilitation of
underground utility infrastructure—bidding on capital or maintenance projects
as it relates to the contractor’s capacity to meet any and all safety,
regulatory, engineering, environmental, legal, manpower, and financial

performance using safety, health, environmental, timeliness, or productivity
metrics on applicants’ competitively bid capital or maintenance projects.
1-23 Provide copies of the last five most recent contract awards for capital projects
valued at over $1 million. Please include the Request for Proposal, technical
evaluations used to qualify contractors—who provided traffic control, paving,
patch paving, utility excavation, and the installation, maintenance, or
rehabilitation of underground utility infrastructure—bid amounts and actual costs
of the awarded contract, and any and all performance evaluations done by
applicants after commencement of the project.
1-24 Provide documents regarding the applicability, or planned applicability, of any
Heath, Safety and Environmental Management System on contracted out
workforce working on maintenance, capital, reliability, or other construction
projects.
Workforce Development & Investment
1-25 Provide documents, including communications, reports, violations, internal
audits, and corrective action plans, related to any investigation or review of
contractor compliance with applicants’ Operator Qualification program.
1-26 Provide a list, timeline, and schedule of any future or active capital projects
valued at over $1 million from 2017 to 2021, and any long-term planning or
supportive documents.
1-27 Describe applicants’ plans to recruit, train, and retain sufficient contractor
personnel to perform forecasted construction activities on capital, maintenance,
and reliability projects.
1-28 Describe applicants’ plans to recruit, train, and retain sufficient Operator
Qualified contractor employees to perform forecasted construction activities on
capital, maintenance, and reliability projects.

1-29 Provide copies of any and all training programs administered by or approved by
applicants for contractor employees.
1-30 Detail annual amounts spent on training for contractor employees from 2010 to
present.
1-31 Provide copies of all collective bargaining agreements, memoranda of
understanding, or other labor contracts in which Avista or its Subsidiaries are
signatories.
1-32 Provide and all documents or a list of beneficiaries of Avista’s $4,000,000 in
annual charitable contributions (funded by both Avista and the Avista
Foundation) as stated in Appendix 8 to the Application.
1-33 Provide any and all documents showing requirements or criteria for grantees
of Avista’s charitable foundation as stated in Appendix 8 to the Application.
1-34 Provide any and all documents regarding Avista’s existing levels of
community involvement as stated in Appendix 8 to the Application.
1-35 Provide any and all documents describing Avista’s past practices concerning
Economic Development as stated in Appendix 8 to the Application.
1-36 Provide a list of all industry trade groups and membership organizations to
which Avista has paid dues within the last three years.
1-37 Provide all documents regarding standards used to determine whether
Avista’s staffing and presence in communities in which Avista operates are
sufficient to maintain the provision of safe and reliable services and cost-
effective operations.
