The PHMSA IA question set and IGC equipment listing were used as a guide throughout the audit to ensure all areas were completed. March 30, March 31,April 1, 2026: (2.5 Standard MG) (2.5 Standard BP) (2.5 Standard AG) (1.0 Standard JB) Met with IGC company representatives in the Boise district office for in brief and to begin field audit and review OQ records for all personnel taking part in the inspection. Regulator Stations: 34 regulator stations were inspected for proper operation, regulator lock-up, relief device operation, cathodic protection pipe-to-soil readings, pipe-to soil interface wrap protection, station warning signs and right-of-way clearances. 1 casing-to-carrier isolation checks, no cathodic protection rectifiers, 10 pipeline patrols, 34 emergency/isolation valves and 24 HPSSs were checked for proper operation, observations listed below. An exit interview was performed following the district inspection with field personnel of Intermountain Gas Company and MDU Compliance Manager Colby Lundstrom. Discrepancies: Regulator Station 48413-RS-59923, the primary regulator failed to achieve lock-up due to significant oil accumulation on the regulator diaphragm. Regulator station 48413-RS-60048, the secondary regulator failed to achieve lock-up due to metal shavings/debris found on the regulator seat. HPSS's with no line markers (carsonite or sticker) 48375-HPSS-68981, 48375-HPSS-62243, 48375-HPSS-62332. Valves: 48413-VLV-29800 was found iced up. Cathodic Protection: A potential reading of—0.62 mV was identified at regulator station 48413-RS-59940, which is below the minimum—0.85 mV criterion commonly used to demonstrate adequate CP. IGC personnel subsequently identified that the test leads inside the Maloney post had been left disconnected, resulting in an inaccurate and inadequate CP reading. April 13, 2026: (0.5 Standard Compliance MG) (0.5 Standard Compliance JB) Inspectors Galli and Brooks made adjustments to NOPV I202603.
