The PHMSA IA question set and Avista equipment listing were used as a guide throughout the audit to ensure all areas were completed. June 8, 2026: (2.0 Standard) (Brooks 1.0) (Green 1.0) Inspectors Brooks and Green travelled to Coeur d' Alene, Idaho. June 9 - 10, 2026: (4.0 Standard) (Brooks 2.0) (Green 2.0) Met with Avista company representatives in the Coeur d' Alene district office for a briefing, reviewed welding records, past year work orders, verified calibrations of inspection equipment, verified OQ's and began the field audit. Eight regulator stations were inspected for relief operation, regulator lock-up, general condition of the station including pipe-to-soil wrap, and cathodic protection. Eight rectifiers, fourteen carrier-to-casings, eighteen control valves and three above ground bridge crossings were also inspected. June 12, 2026: (2.0 Standard) (Brooks 1.0) (Green 1.0) Inspectors Brooks and Green traveled back to Boise, Idaho. June 15, 2026: (0.5 Standard) (Brooks 0.5) Wrote inspection report, completed audit forms. June 16, 2026: (0.5 Standard) (Green 0.5) Wrote inspection report, completed audit forms. Discrepancies: At regulator station 626 the relief failed to reseat after relieving at the correct pressure. The downstream pipe was protected from overpressure. Crew fixed on site and successfully re-tested.
