Inspection Summary
Audit consisted of IPUC inspectors Brooks, Galli, and Green inspecting field installed equipment for the
Kellogg, Idaho inspection unit. The PHMSA IA question set and Avista equipment listing were used as a
guide throughout the audit to ensure all areas were completed.
June 11, 2026: (3.0 Standard) (Brooks 1.0) (Galli 1.0) (Green 1.0)
Met with Avista company representatives in the Kellogg district office for a briefing, reviewed welding
records, past year work orders, verified calibrations of inspection equipment, verified OQ's and began the
field audit. Six regulator stations were inspected for relief operation, regulator lock-up, general condition
of the station including pipe-to-soil wrap, and cathodic protection. Four rectifiers, seven carrier-to-
casings, twenty-five control valves and five above ground bridge crossings were also inspected.
