Observations included (4147 E Bogie Drive APT 101): Safety equipment, JSA, verified operator
qualifications, verified equipment calibration, correct trench depth, Lyco fitting, EFV 1200 pre-installed
per maps, tracer wire, bedding with sand, successful pressure test and squeeze off. Quizzed employees on
applicable AOCs for squeeze off procedures. Quizzed employees on applicable AOCs of utility separation.
All observed procedures were followed in accordance with Avista procedures.

qualifications, verified equipment calibration, correct trench depth, Lyco fitting, EFV 1200 pre-installed
per maps, tracer wire, bedding with sand, successful pressure test and squeeze off. All observed procedures
were followed in accordance with Avista procedures.

qualifications, verified equipment calibration, correct trench depth, Lyco fitting, EFV 1200 pre-installed
per maps, tracer wire, bedding with sand, successful pressure test and squeeze off. All observed procedures
were followed in accordance with Avista procedures.

qualifications, verified equipment calibration, correct trench depth, Lyco fitting, EFV 1200 pre-installed
per maps, tracer wire, bedding with sand, successful pressure test and squeeze off. All observed procedures
were followed in accordance with Avista procedures.
