Additional Comments:
IPUC staff traveled from Moscow to Coeur d'Alene, 19 May 2026.
Observations included (11494 N Caravan Ln) (Hayden): Safety equipment, JSA, material verification,
verified operator qualifications and correct trench depth. Lyco fittings, bedding and backfill. Customer had
not completed the trench so that's all that was observed. All observed procedures were followed in
accordance with Avista procedures.
Observations included (6143 W Pelican Loop (Rathdrum), 3670 N Heton St and1528 N Wilhem Way
(Post Falls): Safety equipment, JSA,material verification, verified operator qualifications, correct trench
depth, pressure test, EFV's, purging, bedding, backfilling, setting meter bars and risers. All observed
procedures were followed in accordance with Avista procedures.
