Additional Comments:
IPUC staff traveled to Lewiston on May 19, 2026, to observe Avista crew replacing 2"plug valve because
it was flanged underground and problems with CP readings.
Observations included Safety equipment, JSA, verified operator qualifications, weld cards, leak testing,
welding, exposed pipe report, bedding and backfilling. All observed procedures were followed in
accordance with Avista procedures.
