Dear Idaho PUC:
Please be advised that Amerivision Communications, Inc. is no longer transacting business in the state of Idaho and
we have surrendered our authority to transact business in the state. The business entity is Amerivision
Communications Inc., and its DBAs are Affinity4 and Lifeline Communications. Per the SOS's office, Amerivision is the
owner of the DBAs (Affinity4 and Lifeline) and when the owner is withdrawn the DBA is no longer in operation.
The Secretary of State in Idaho issued a Statement of Withdrawal for Amerivision Communications Inc. on April 30,
2024.
Since 2024, we no longer have subscribers in the state of Idaho and we wish to withdraw any/all PUC authority and
tariffs if applicable.
