NOTICE TO OPERATE AS AN INTERCONNECTED
VOICE OVER INTERNET PROTOCOL SERVICE PROVIDER IN IDAHO
TWC IP Enabled Services, LLC ("Applicant"), pursuant to Rules 201-202 of the Idaho
Public Utilities Commission Rules for Telephone Corporations Subject to the Regulation of the
Idaho Public Utilities Commission Under the Idaho Telecommunications Act of 1988, Idaho
Administrative Procedure Act § 31.42.01, and Section 62-604(1)(b) of the Idaho Code, hereby
submits this Notice to the Commission to Operate as an Interconnected Voice over Internet
Protocol ("VoIP") Service Provider in the State of Idaho. In support of its Notice, the Applicant
provides the following information:
a. The name of the Applicant and the business name of the Applicant if it does business
under an assumed business name.
The legal name of the Applicant and name under which the Applicant intends to do
business is TWC IP Enabled Services, LLC. Attachment A includes Applicant's Certificate of
Existence issued by the Idaho Secretary of State.
Applicant is an affiliate of Time Warner Cable Business LLC and Time Warner Cable
Information Services (Idaho), LLC, both existing registered telephone corporations in Idaho.
b. The address of the principal place of business of the Applicant, and, if there is a
principal place of business in Idaho, the address of the principal place of business in
Idaho.

of its affiliate, Time Warner Cable Information Services (Idaho), LLC, within the State of Idaho.
Applicant will use the existing network infrastructure and distribution plant owned by Charter
Communications, Inc., and its subsidiaries, to provide service in Idaho. Attachment B includes a
map of Applicant's proposed service area.

consumer inquiries, complaints, etc., by the public.
With respect to consumer inquiries and complaints, the public may contact the following
