Additional Comments:
Observations of IGC crew included (California/Colorado/Nevada&5-8,h and Utah&4th, Gooding, ID,
Safety equipment, multiple times crews were not wearing proper PPE, had not completed JSA or correctly
covered needed items, valid dig ticket, locating, verified operator qualifications, verified equipment
calibration, correct trench depth, welding on transition fittings, observed/verified proper rollback of
completed 4", 2" & V butt fusions, installed 200' of 4" PE pipe, 1500' of 2"pipe and 1000' of V pipe,
pigging, tracer wire and bedding with sand, backfilled with native soil, compacting, boring and rock
sawing. Quizzed employee on applicable ADCs for rough handling times.
Twin Falls IGC leadership met with KV leadership to discuss shortcomings on wearing proper PPE and
not completing JSA's correctly.
